Hi all, hope you're all well and staying strong.
I have been diagnosed with stage 3b lung cancer in January 26 I start my treatment on Monday 9th March. I am sorting my benefits out. I have been asked about a SR 1 form which I've never heard of. Does anyone know if I qualify for one ???

A SR1 form is only issued by your medical team when you are expected to not live past 6-12 months. I would expect this was just a standard question from the benefit team.
I'm a survivor of Stage 3B, my last treatment was Durvalumab in April 2020, and I was discharged from further surveillance as no evidence of recurrence by July 2024. So always have hope <3
Iam so pleased for you I did not think they would give me treatment as Iam 81 but very healthy mine was missed on a X-ray 6years ago
