Keytruda immunotherapy

Good evening,

I would be interested to know if delaying/interrupting the three-weekly cycle of treatment has any adverse effects on the effectiveness of the drug.  More specifically, if two consecutive treatments are spaced at four weeks rather than the normal three?

I am just starting my journey with NSCLC and have completed two treatments to date.  My third treatment would have taken place on Good Friday so it has been re-scheduled for the following Friday, one week later. A four-week gap.

My fourth treatment will then fall on a date when I will be on holiday.  I am required to go for a blood test in the morning two days before my treatment appointment (Wednesday).  At the moment I will not return until the Wednesday evening so there is insufficient time for blood test before my normal appointment on a Friday afternoon.   

The obvious answer would be to move my treatment to the beginning of the following week (Monday), however I know that the oncology unit is always extremely busy on Mondays, so it is probable that they would prefer a Tuesday appointment.  In addition, I work full-time and although it is no problem whatsoever for me to be absent; I'm quite conscientious and would like to have as little absence as possible therefore a Friday appointment is ideal.

I do not know if it is possible to ask specifically if the blood test can be done the day before treatment?

A further option would be to return from holiday one day earlier but because I am going to be doing a lot of travelling I would find it very, very tiring to shorten my trip.

I would appreciate your thoughts.

To digress slightly, so far I have had very minor side effects from the Keytruda.  I do feel slightly tired and have a very slight headache when I get home after treatment (but it is a 50-mile round trip for me).  Going to bed for an hour or so seems to work.  No problems at all on the following two days.  On the third day after treatment I do have very, very slight nausea, but this seems to be caused by wind, a good belch or so sorts it out!  I do not feel that I need any anti-nausea medication.   I hope that I continue to be fortunate with the side effects and that all those in this lung cancer group who are starting with pembrolizumab will be fortunate too.

    regards your question. my doctor allowed me to go a day early for bloods as i was at the hospital. told me to say doctor ### said it was ok. unsure about treatment  times   i am on a different  Immunotherapy but it has the same time tables

    Hi Artemisa

    I do not believe there will be any impact on the treatment for a 4 week gap at all. I know chemotherapy cycles can often be delayed a week or so here and there as the blood levels aren't high enough for the next cycle. 

    I would see if they can do the blood tests for the following cycle the day before and try and enjoy that holiday! It is lovely to have something to look forward to.

    Thanks for your reply.  It's not that I need to go early for my blood test but that I need to go late and I'm not sure if this will be possible.

    I know that treatments are sometimes delayed because of unsatisfactory blood levels, however I was interested to know if two consecutive delays would have a deleterious effect on the success of the treatment.

    I will investigate if it is possible for me to have blood test done just one day before my treatment as this would be the best solution.

    Hi 

    I used to have my bloods done they day before. Also had treatment delayed for holidays etc. 

    All was good still. Did not effect treatment or how well it worked. Hope the same for you too

    I think I might be insistent that it should be done just one day before.  I did agree to a complete week delay due to the Good Friday holiday so perhaps they will be able to accommodate me..

    in your position i would ring my cancer nurse . give her the details. mine is back to me within a day.  all the best

    Hi artemisa 

    i often had to miss a week of treatment and on one occasion it was almost 5 weeks from the previous one.  When discussing my concerns with my onc I was told it was nothing to worry about.

    As for the blood tests, if I couldn’t have the results back on time for any reason, the treatment centre would do them on the morning of my treatment.  It meant waiting around at the hospital a lot longer than usual, but I thought this better than missing a treatment.

    Hi artemisa   i read on a american site that there can be up to a 12 week gap in treatment.   need check out with nurse
