Stage 4 kras mutation advice Re treatment

Quick update - mum, 77, has been diagnosed with stage 4 lung cancer last week. She is bizarrely well, except breathless and tiredness( the diagnosis has come as a devastating shock!)

She was due to start chemo and immunotherapy on 28/2.

we got a call yesterday to say tests had discovered a kras mutation plus 100% pr1. The oncologist presented 2 new potential treatments

1. See if there is a kras clinical trial available in Newcastle or manchester as there are none currently open in leeds

2. Targeted immunotherapy and no chemo yet

We may not even get the option of a trial but in case we do, I just wondered if anyone had any experience or insights into the above. It’s all quite confusing and I’m keen to try to make the best decision possible. I’m also mindful that diagnosis max tests etc have been going on since late Dec so we need to urgently get going on treatment. Any insights or tips are gratefully received! Thanks

  • Hi  cancer treatment is such a rollercoaster, and there are always twists and turns along the way to get us to the right treatment. Just when we think we know what is happening, a spanner gets thrown into the works.  My cancer journey is different to your mums, but I hope you don't mind me sharing my thoughts with this with you.

    Trials are fantastic, they are so important for the future of cancer care, so many treatments would not be available now without the trials. One of those trial drugs I spoke about back in 2021 when it was being offered to patients with the Kras mutation. I will put the original report link below for you, but remember it is from 2021 so has come a long way since then.

    I wonder if this is the targeted therapy the oncologist has in mind for your mum? As there are currently no trials available, I would think the immunotherapy is the best option. This does not mean that mum could not join a trial further down the road once one becomes available. It just means there may be further options ahead if the immunotherapy does not work.

  • Thanks so much! Really interesting reply. I think the trial was for first time treatment BUT we have later found out there are none available in manchester or Newcastle so she will start immunotherapy next week!

    Very interesting! I have stage IV lung cancer- with a currently undruggable KRAS mutation. It’s one of the rarer ones (typical me, attention seeking, lol!) Hence I’m on immunotherapy with chemotherapy, about to have my 9th cycle. 
    my first scan 3 months ago showed brilliant results. The immunotherapy is the larger part of the treatment; I started on 2 chemo drugs with it but now that’s dropped to one. 
    I had my latest scan yesterday so am waiting; but I’ve heard rave reviews about the immunotherapy so I’m a fan and so is my oncologist. Recommended from me! Good luck and keep us posted.

    I also have a rare/unusual mutation of the KRAS, so wasn't eligible for a clinical trial.

    Now having maintenance chemo and immunotherapy. Mixed results so far.
