Hello Advice on grade of tumour.

Hello there, I'm new but have lurked these passed few weeks and gained a lot, so thank you.

Following at CT scan which found a tumour on my right kidney I had it removed in December.

I went to see the consultant yesterday and got the histology results. And the all clear. Follow up scan in May and regular scans tailing off.

The only bit I'm concerned about is the grade of tumour, grade 4, the most aggressive. This is the grade not the stage. It is a stage 1 cancer.

I asked about immunotherapy but he said that would be several stages down the line. I could never be a candidate for chemotherapy due to impaired kidney function.

So in summary I'm very pleased with the results but I am concerned about the aggressive nature of the tumour.

Any thoughts?

  • Just my thoughts, I have no medical knowledge, but whatever the grade of your cancer, it's gone now so you are cancer free and hopefully forever.

  • Did they tell you the type of cancer?  I believe aggressiveness is linked to type.  There are: 

    Common Types (Renal Cell Carcinoma Subtypes)
    • Clear Cell RCC: Most common (70-80%), with pale, lipid-filled cells.
    • Papillary RCC: Second most common, forming finger-like (papillary) growths.
    • Chromophobe RCC: A rarer form with larger, darker cells.
    • Collecting Duct Carcinoma: A rare, aggressive type forming irregular tubes

    Other  Kidney Cancer Types 

    • Transitional Cell Carcinoma (Urothelial Carcinoma): Starts in the renal pelvis, behaves like bladder cancer.
    • Wilms Tumor (Nephroblastoma): Primarily affects young children.
    • Renal Medullary Carcinoma (RMC): Very rare, aggressive, often in young adults with sickle cell trait.
    • Renal Sarcoma: A very rare cancer of the kidney's connective tissues or blood vessels.

    If you kow the type it would help you to connect with others and compare experiences etc.

    I take the approach with mine, that I can't influence whether it grows again, so don't worry about it.  I know there is a plan should it return.  I came to terms with my situation.  It took a few years but I'm prepared for any scenario, or so I believe!  But happy not to test it out!

    I had a full R neph in December open surgery. Mine is a clear cell. I have a leibovich score of 9 and small imdeterminate lung lesion on R lung and been offered pembro immuntherapy. I am assuming my grade is 4. I have not actually been told.

    I am thinking you may not require immunotherapy as they have removed all the cancer after surgey and not showing any concerns with previous scans and the cancer was below 7cm? 

    I would see this as a big win Trophy

    As others have said I am also no medical expert, and that is why we should usually listen to them Grin

    I had similar right removal full nephrectomy and Timor had grown into main artery. See bio. Mine was grade 2 but was offered immunotherapy which I declined. Hope it is still a tool of required further down the line. I can only presume they are very happy with your surgery result. Scans become a worry but they are important to provide confidence in your recovery. I am 18 month's post surgery now and so far so good.  Easy to say and the niggle is always there but don't let worry of return get you down. Positivity can only help your wellbeing.
