I think there ought to be a questionnaire here for all patients to fill in. The questions should relate to the treatment and after effects. Your age and how long ago the treatment was. How you are coping with eating sleeping breathing walking, toilet functions etc. Of course many problems can come for other reasons like old age, even if they start just after treatment. I have in my regular posts here answered most of them how they pertain to me. Also if there are any improvements that can be made, I have mentioned plenty. This is just a general idea of what to ask but can be taken much further.
I'll pass this on to the mods
But there is some information on the site. You just need to know where to start and THIS is as good a place as any.

Not sure a questionnaire is good - what do we do with the answers...? However, I have thought for some time that a Wiki would be useful with people contributing to the common questions so that someone has a place to go and research answers. It would probably be more detailed and intimate to a patient's journey than the Macmillan version, which by its very nature has to be more general in the H&N field, but all the same very useful.
