Hi all.
I'm wondering if anyone has any experience with granuloma or sarcoidosis after treatment?
I've had six cycles of R-CHOP for DLBCL/Follicular Lymphoma, followed by two treatments of straight Rituximab (with my third this week)
After my first Rituximab, a scan showed new spots, so they stopped treatment, booked a biopsy and found granuloma.
A scan three months later was exactly the same, so they went on the assumption that what I have in the other spots (liver, spleen, one lung) are also granuloma.
My most recent scan (three months after the last one) showed even more new spots, so the oncologist now wants to biopsy a different organ.
My question is, if anyone here has had granuloma/sarcoidosis pop up, has the number of spots increased at all?
The oncologist says the new spots could be granuloma, like the last one, but they could also be relapsed FL (though I have none of the symptoms like fever, night sweats, weight loss)
Any insight into this would be much appreciated. My next step is a CT scan and a biopsy.
Hi Acacia, different NHL and treatment but a scan following my second Stem Cell Transplant showed a ‘growth’ on my lung (I had to google granuloma)
I was on the CT table all ready for a CT guided biopsy but the surgeon looked at the rescan getting ready for the biopsy and she said it had gone down slightly so did not want to do the biopsy so no idea what it was.
Due to various infections and hospital stays a number of scans has shown that the lump has not changed so I now have put this all on a high shelf. All the best.
I'm really hoping that happens for me. I had a biopsy last time new spots popped up and it was granuloma. I'm hoping for more fo the same. The oncologist is sceptical because they're new growths after two stable scans.
Do you know if a CT scan can tell if something is lymphoma or inflammation? I'm hoping that it will give some indication of what it is. I'm booked for it on Wednesday.
My gut reaction is no, but I am no expert

Thanks. Fingers crossed... With the last one, they did an ultrasound and couldn't find most of the spots they wanted to biopsy...
Scans do show some things, but I cannot say about inflammation versus lymphoma. I was told my big hip bone lymphoma wasn't a chondrosarcoma after my first CT, which was a moment of unalloyed pleasure at the time!
I hope your investigations are reassuring too. It's only natural to worry about alternatives in our position, but your team sound like they're keen to make 100% sure for you.
