I was diagnosed last September with metastatic breast cancer with spread to my right lung. I was half expecting to be diagnosed at some point as both my mum and my maternal grandmother had breast cancer, but just wasn't expecting the diagnosis to come at 59...a lot younger than they both were at diagnosis.
Having been seen back in January 2025, biopsies then, showed fibrocyctic changes, I was put back into the 3 year cycle for routine mamogram. Fortunately, I didn't wait that long before referring myself back to the clinic in July 2025, with stage 4 diagnosis coming in September.
I haven't had surgery or IV Chemo but I was started on Letrozole initially then Ribociclib. The Ribociclib dose initially was 600mg but after a couple of episodes of neutropenia my dose was reduce and has subsequently been reduced again. I am now on 200mg daily. However, the pattern seems to be that my white cell count falls to about 0.8 after one weeks break then climbs back to 1.5 after 2 weeks break. So my question is, is there anyone who takes Ribociclib on a 5 week cycle, 3 weeks on and 2 weeks off? Are there any benefits/problems with this regimen?
Hi there, I'm one of the champions here in this group and whilst I’m not on that treatment, I just noticed you've not had any responses yet. My answer will bump your question back to the top again and I hope someone with the right experience will be along soon. Best wishes
Hi Phoeb Ns26. I had lobular breast cancer in 2013 aged 47. In September 24 was diagnosed with secondary lobular breast cancer aged 59. I have had no surgery or chemo either. I am on exemastane which is like Letrozole and ribociclib and denosumab as have bone mets. I also had very low neutraphils on 600mg ribo it has now been reduced to 400mg which I seem to be coping with. My cycle is still 4 weeks with three on and one off. I have not heard of anyone on the 5 week cycle. Have you asked your oncologist about benefits/problems? I try and research as much as I can but some of the technical stuff goes right over my head. How are you coping with this cycle?

Hi Lee, thanks for your response to my post. I have asked my oncologist about changing to a 5 week cycle but she wasnlt aware of anyone following that regimen so said she would consult with her colleagues and get back to me. I have asked on various forums and it would appear that there are all sorts or regimens out there that people are following, one of them being 3 weeks on and 2 weeks off. Will just have to wait until I chat with the oncologist again. Apart from my neitrophils dropping, I have no other side effects...thankfully.
Hi Phoeb glad to hear you have no other side effects. Just found out that another lady on one of the forums is on palbociclib and she has just been changed to three on and two off because of her neutrophils. I hope this works for you.
