Oncotype score chemo or not

Hi there I am facing a very difficult decision this week.  I was diagnosed with breast cancer this year initial biopsies stage 1 18mm grade 2 er pr postive her2 negative was told don’t worry this is the most common and treatable cancer.  After my lumpectomy stage 2 29mm and grade 3 no lymph node, so tumour was sent for oncotype scoring.  My score is back it’s been graded 24 9% recurrence risk <1% no real benefit of chemo however my Dr is recommending I get it but has let me decide if I want it or not.  I am really struggling as I want to make the right decision but I don’t really understand as my score states no benefit but they are still recommending.  I am terrified at the thought of chemo if it was clear cut then obviously I would go right ahead and get it done but it’s not.  I know this is a very personal decision I am 54 post menopause is there anything online or advice anyone can give, has anyone been in a similar situation is there anything I could read to help me decide ?

  • What a decision to  have to make, I really feel for you. My cancer last year was very similar to yours, very er and pr positive, her2 negative, 28mm lump and no lymph nodes affected.  So I had lumpectomy and a re-excision 6 weeks later to get a better margin.   I then had 5 sessions of radiotherapy.  Luckily my  onco score was 5, so chemo wasn't recommended and therefore thankfully no decision had to be made.  I was 65 and am on Letrazole for 5 years.

    Can you speak with the people at Macmillan to chew things over, and get more of an explation from your hospital team as to why they are recomending chemo.  Maybe they are just being ultra cautious in giving you as much treatment as they can? x

  • I didn’t have chemo or an Oncotype test but was told right from the beginning that if my cancer had been higher grade than the initial biopsies showed (mine was Grade 2) or any nodes involved at all that they would recommend this because of my age (I was 46 at the time). It seems it may be standard practice to offer chemo for Grade 3, but I may be wrong. Perhaps you can call your oncologist next week to discuss again and ask them what they would do in your position? x

And hope your treatment is going well Lots going on in my mind I do think at these appointments although I had everything written down with questions I wanted to ask you become so overwhelmed and the conversations can go down various routes so you loose track. Think I need to talk to someone again I am swaying away from chemo but the grade 3 is bothering me !!

    In December I was diagnosed with Invasive Lobular, Grade 3, 50mm PR Positive HER2 Negative, 0/2 lymph nodes. I had a mastectomy in January. Following this my Oncotype score was 17 so my oncologist advised chemo not required. 
    I’m now on 7 years Hormone therapy (Letrozole) and 3 years Targeted therapy (Ribociclib). 
    I am 57 post menopausal. 
    I’m sorry you have this difficult decision to make. Why don’t you discuss further with your oncologist. Hopefully they can explain it further to help you make the right decision for you.

  • Hi, mine was grade 2, altho a bit bigger than they'd initially thought. 

  • Hi. My breast surgeon said it was grade 2 but the oncologist treated as a 3 due the size (50mm)

    I am in a similar situation.  I was diagnosed with stage 3 triple negative breast cancer six weeks ago.  Had surgery and have my oncology appointment next week to decide on chemo, followed by radio.  Thoughts of chemo is daunting.
