Newbie looking for help with Docetaxel and Phesgo

Hi, i was diagnosed with Grade 3 ER negative, PR negative, HER2 positive breast cancer in April this year. I have had 4 rounds of EC chemo and my first of 4 Docetaxel and PHESGO 3 weeks ago. 

I just about managed the first 4 rounds, the pain was managed with med patches and Orimorph and I struggled with the oral thrush and mouth ulcers even though I was prescribed Nystan and specialist mouthwash. The Docetaxel and Phesgo however has completely floored me.

I had the treatment on 1st August and have had awful slimy stuff on my tongue, oral thrush and ulcers in addition to a swollen and burning tongue. I was unable to speak until 2 days ago and am still being sick at night and sleeping sitting up as I'm terrified I will choke at night.

I cry at the thought of my next chemo (due next week). Has anyone else had the same? Can the chemo be reduced to manage the side effects or is there an alternative?

23/08 Update

Thank you everyone for your support. I spoke with my oncologist on Monday.  The upshot is she said herself that I needed more time for my recovery so my next chemo has been postponed to Wednesday 30th August. In addition she said my side effects were down to Docetaxel not Phesgo so the next Docetaxel will be reduced by 20% and my side effects monitored- She said I needed to flag these as soon as I had concerns and not to wait. I was also prescribed different meds to help with the slimy tongue/ulcers. 

The final positive was that my chemo is working, the MRI taken after my 4th chemo shows my tumour has shrunk by 8%, not quite 1/3rd but really good news and just what I needed to spur me on.

    Sorry to hear you are not doing well. I think you should speak with your BC nurse and your oncologist and let them know about all you side effects. They might reduce your dose or change your drugs if appropriate, but you should let them know. I hope you'll feel better soon!

    I had x6 TCHP which finished December 2021. For me the first one was definitely the worst. I had my 5th and 6th doses reduced which did make a difference. It’s definitely worth discussing side effects with your team, they should be able to give you something different for the sickness and get on top of things for you. I cried before each treatment and found  the whole thing horrible but I’m still here getting on with my life again, back to normal fitness, hair has recovered and I think I have more of it than before!

  • Hi   definitely ring your unit and speak to team they will not want you to suffer with extreme SE . Your chemo can be reduced or changed I was lucky my Onco recognised on our first discussion that because of an existing condition I was not a candidate for Docetaxel .

    Plan was changed and I had EC X 6 AND 18 Herceptin . I also had the option , because of cumlative SE , to delay some treatments .

  • Hi Margaret, wow you really have been through it. I read your profile as you suggested and it gave me so much  hope. I was diagnosed with polymyalgia over 10 years ago even though I didn't fit the profile for the illness. I think that has caused some additional issues as my body flares up with steroids!! I will definitely do as you suggest and thank  you for your support x

  • Sorry to hear you’ve been diagnosed with Breast Cancer and you’re suffering with treatment. 
    I was diagnosed on the 24th July with exactly the same as you PR Negative ER Negative and HER positive l

    Ive had my first Chemo session on the 11th August (Chemo first the surgery) so I’m 9 days post Chemo and like you I’m really suffering with side effects iI think I have everything going, from sore moth ulcers, I thought I’d got away with the sickness but that arrived on day 6 and now day 9 I have diarrhoea, is this ever going to end!
    Please tell me I will feel better as not sure I could cope an another 5 rounds.

  • Hi Sasha, sorry to hear about your diagnosis and how you're feeling. The first 1 chemo wasn't too bad but as I also have polymyalgia the pain was excruciating.  My GP switched me to medicine patches part way through and Orimorph which definitely helped. I can't lie though,  the mouth issues have progressively got worse each time and the 5th chemo has been the worst. Given all the advice and support I've had since my post, I will be speaking with my oncologist tomorrow so will update after that. I also found, during my 5th chemo, getting my husband to speak with my pharmacist helped too as she was able to offer options for my rash and mouth ulcers. I think because I was unable to speak until a few days ago, (due to mouth ulcers and swollen tongue) it probably became worse than if I had flagged it earlier. You will get through it.

    Just don't do what I did and assume that is how it should be. This forum has definitely taught me you should tell your nurses and oncologist sooner xx

  • Hi. Chemo can most definitely be reduced. I sailed through four cycles of ec then first cycle of docetaxel I got sepsis and ended up in hospital. My oncologist reduced my dose to 75 percent and although I got paraflu second cycle I had nothing connected to the docetaxel. So definitely speak to your oncologist.

  • I am/was hormone neg, her2 pos, finished treatment December 2021. I found I got into a pattern of symptoms as each cycle went on and started to pick up gradually after about day 10. The last week before the next cycle I felt fairly ok and was out with the dog etc. 

    Its useful to keep a diary of symptoms each day for each cycle.

    It is awful but it does pass and you will be out the other side. I read a blog by Irish girl who was a couple of months ahead of me with TCHP on this site which I found helpful.
