Mind in overdrive

I was diagnosed with sigmoid cancer and had my op in March . I also had a pre cancerous polyp on the entrance to the small bowel . At the MDT meeting it was decided that I should have the sigmoid tumor dealt with first rather than having a sub total colonectmy. So I had instead a higher anterior resection with a view to removing the polyp layer on . Well yesterday I had a colonoscopy and the polyp in Jan this year was 4mm yesterday it was 13mm . I’d say that was a pretty aggressive growth ?? The surgeon didn’t say much but he has classified it as Paris lla and pit type llls which is exactly the same as the sigmoid tumor - my mind is going overboard with thoughts . My gut is telling me that I’ve got another tumor .
Because of where this polyp is on the ileocecal valve it’s not clear if it can be removed with EMR colonoscopy or whether I’ll need a right hemicolectomy. The surgeon says he will consult with colleagues . I think I’ll need to get some clarity from my nurse on Monday ! To make matters worse my chemo has been stopped as I contracted Covid at the beginning of August and the oncologist doesn’t want to start chemo again untill my lungs show improvement ( I’d had 4 cycles of cape only )

has anyone else had a polyp grow so much ? Does put type llls indicate a probability of malignancy ?

  • Hi

I remember you referring to the second polyp a while back ! I had a look at the Paris classification but it seems to indicate the appearance and pitting also comes into play ! However I can totally understand why you are concerned especially as it is a similar presentation to your tumour !

Definitely need the support of your team but I think my mum had completely breached the Paris classification as her tumour had banded and folded ! However it was a very straightforward resection in right side ascending for her !

  • Hello Takayasu, after my recent colonoscopy I got inconclusive, so it was repeated and mine had that Paris pit in the description. They thought it was a deep submucosal invasion.a lot of other decrpitive medical terminology. Anyway after being told it was cancerous  twice in the conversation following theMDT and still inconclusive after the 9 biopsies that they took. I’m not watching and waiting, I’m seeing a surgeon and still am going to be treated as if it is a cancer. In the report they mentioned surgery or something called an endorotor. I had chemoradiation last year and got complete response last year. Take care Nelly55 xx

  • Hi , thanks for your reply . Yep this polyp was biopsied in jan as a pre cancer polyp . Some of the  surgeons in the initial MDT meeting g wanted me to have a sub total colectomy but my surgeon thought it was best for me to get my sigmoid adenocarcinoma out first then revisit this polyp later . I don’t think he would of thought it would grow so much in 7 months ! Because it’s on the ileocecal valve I always knew that it would probably need surgery but never imagined it might turn into cancer yet ! I spoke to my nurse this morning who said she would put the colonoscopy report under my surgeons node and she thinks it will be discussed at this fridays MDT meeting . I’ve had 4 cycles of chemo  out of 8 which makes me wonder if this polyp is unresponsive to the chemo too - it will be a long week for me I’m sure .

Are you team happy for you to proceed to straight to surgery?
