G3T1 with Squamous Differentiation?

Hi Folks

My first OP appointment ~2 weeks ago gave the news of  multifocal G3T1 with potential squamous differentiation which was a little unexpected. The flexible cystoscopy showed 3 small (<10mm) smooth lumps with a few capillaries wafting on the surface. Didn't look that scary, but this shows that looks can be very deceiving!

OP options given by consultant were BCG or Cystectomy, with heavy leaning towards the latter. I have another TURBT booked next week and can't wait to get the pathology results on this and choose treatment path. Although I said that I would prefer to conserve the bladder with BCG at the appointment, further reading is pushing me towards cystectomy with ileal conduit. Was offered neobladder as I am "young (63 next week) and fit (albeit the big belly)" but at the moment I would prefer things as simple as possible, with quickest recovery so leaning towards conduit.

Anybody had this or similar diagnosis, and if so, what was your choice, and considerations to get there .. and comments on how it went please, including recovery time?

I fully understand that we are all unique / different and make our own decisions, so no right or wrong answers. The decision I make will be solely mine, and I don't look back to regret my decisions as they were all right for the time that I made them.

  • Hello Pegasus, I hope all went well with your 2nd op. I had my re-do turbt about 4weeks ago. had my appt at clinic today and they confirmed it was high risk non muscle invasive bladder cancer. surgeon said he wants me to start BCG  and was very positive that it was the right path for me to go down. I was T1G3 with CIS . I am going to trust him and give it a go, my eyes wide open as to the down side of BCG, but wont know until I start, so praying it will work for me, and I wont have too many side effects.   • Well it's working for me so far a year on so I can only hope and pray this continues, I will keep going with the Bcg for now, good luck on your decisions and journey. It's so tough but sometimes it all seems ok

  • Taken the decision following long chat with consultant at OP appointment following 2 TURBT's and OP appointments .. and its RC for me.

    The variant pathology weighed heavily on my decision, alongside better chance of  a "cure" with fewer visits to the hospital over the next few years. 

    Just waiting for appointment with surgical team to get the next stage rolling. How long is normal from decision to operation?

    my TIRBT went really well no recurrence and now awaiting results. Please keep me informed as to the BCG.

    I was only given the RC option, well I could have had radiotherapy, but the recommendation was an RC.

    I know it all seems very daunting, but life after the RC has been fine and you are given the option of a neo bladder. I chose a stoma and life has been fine. If you choose the RC route it is quite drastic, but there are fewer follow up procedures. However, I would definitely wait to see what your results are and what your Medical Team suggest.

  • Waiting time for op will depend on hosp schedule, having a bed available post op etc. Probably a few weeks. Because I was keeping options open between stoma & neo, I had another TURBT & CT scan between saying yes to surgery & getting a date. Ended up with just 1 week's notice. Gives you time to prepare, et as fit as possible, line up clothing & entertainment/activties for post op recovery period & arrangements for help with housework, laundry, shopping & cooking if necessary.
