Recurrence and doctors recommending APR (NEW UPDATE IN POST)

65yo female

Diagnosis Squamous Cell Anal Carcinoma 7/24 (cancerversary is tomorrow!) Stage 2b

Nigro protocol chemoradiation ended 10/24

Restaging PET 4/25 shows mets to liver

Liver resection 5/25

ctDNA test 7/25 (waiting for results)

New lesion on anus 7/25 with excision and biopsy shows return of SCC in anus 

Both surgeon and oncologist are recommending APR.  Surgeon suggested getting second opinion from MSK, Mayo, etc.  

I said I probably didn't want APR, but surgeon explained how bad it would get if I DIDN'T have surgery.

Just putting this out there so others have info

*NEW UPDATE- Today 7/28/25 the oncologist said he presented case to Tumor Board and consensus was to first start chemo/immunotherapy (carboplatin/taxol) (Retifanlimab) next week, and postpone surgery and hope for response so we can switch to immunotherapy only.  I also have a call into nearby Cancer Center for a second opinion (suggested by surgeon).  I had wrapped my head around surgery, but glad for this reprieve, hoping it's successful.

    I can't imagine just how you are feeling right now x it's a huge, life changing surgery... but if it means you survive? I guess any one of us would do what is necessary. It's certainly a fear of mine that if the standard treatment doesn't do the job, what next? I hope it all goes well for you and that you find a new normal that brings you a pain free, cancer free life! And that you can live it to the fullest Purple heart

  • Thank you, Jenna.  I am actually already thinking about after surgery and how I will do the things I love like running (which has gone by the wayside since diagnosis) and looking forward to the future with family and friends.  If this surgery can do that, I will take it!!  Thanks again for your encouragement!

  • I see that  has kindly signposted you to the Bowel (colon and rectal) cancer forum & another one that may be useful is the Ileostomy, colostomy, urostomy and any other stoma support I think so much of when you’re facing any big surgery is getting yourself mentally prepared as possible & you sound as though you’re doing this.

    I am very sorry to hear of your recurrence and that APR surgery has been recommended to you.

    I too have had APR in 2023 and honestly my life is totally normal now except for the stoma. I actually prefer life with a stoma as I have Crohn’s disease which has been helped by the stoma too. As has already been mentioned it’s tough surgery and i had a few problems with healing but once you are on the other side I hope you will have the same positive experience and find life is pretty good!
