Travel Insurance UK expat in Malta

Hi

I have been searching for travel insurance to cover my wife. We are from the UK but have lived in Malta for over 4 years. There are no local companies who offer travel insurance for people with pre existing conditions and most of the good UK ones seem to only offer cover for UK residents. Does anybody know of any companies I could try?
