PIP Zia very complicated and you do need help from experts to fill in these forms.
You may find our various Macmillan Support Line Services on 0808 808 00 00 to be helpful as you can talk to a friendly person that can help in lots of ways.
You could also check out if you have a Maggie’s Centre near you as they can help with these issues.
Simon I meant to add with a DS1500 it should be straight forward and if you read the guidance you will see you will be fast tracked and should have a payment within 2 weeks, so get some help and hopefully you will be sorted. If you read the CAB link it tells you you don't need to complete the form under the section how to claim when you have a DS1500 which is what I thought.

Usually the advice given by Macmillan advisers re PIP is very good. On this occasion, however, your dad has been given incorrect advice and information.
I have PIP with advanced care and mobility following a SCT for AML. The latter gives me access to a Motorbility car. I was awarded it on the basis of the information in my form. I didn't even have a medical assessment.
I used to work for CAB and you need to ensure, in the first case, that you have requested a Mandatory Reconsideration. The next step would then be to appeal. 80% of PIP decisions are overturned on appeal.
You need to prepare for the MR with plenty of information about what has been overlooked and what the DWP has failed to take into consideration. Go to CAB and ask for assistance with this. I have said elsewhere that one of the biggest mistakes made by applicants is not to put sufficient information in the application. Prepare carefully with notes and think laterally.
If you think that an issue may be that insufficient information was put in the original application, you can reapply. This will not negate the passage of the original application.

Yes - with a DS1500 the passage of a PIP application is usually very rapid.
