Confused

My dad has recently been diagnosed with Transitional Cell Carcinoma, he has cancer in his kidney. Due to his other medical conditions (COPD being one) and his general health and mobility he is unable to have any treatment or surgery so has been told he is terminal. He is on palliative care and is currently still in hospital. He is under Urologist consultants and kidney surgeon and had not been seen by a oncologist although I believe one has been present at MDT meetings.  I am confused a lot of the time and I don't feel as a family we are supported. I am a bit confused about what Palliative care actually involves once dad is home and who will deliver it.  He will have carers to help him with daily tasks but what about medically?

Just so hard to see and talk to doctors as they are rarely available and the team caring for dad in the hospital seems to constantly change. I am just really overwhelmed.
