TSH suppression: How much is too much?

Hi everyone, hope you’re able to help me here.

As I’ve posted about on here before, I was diagnosed with PTC in November 2020. I had a TT the following month and RAI in February this year.

Ever since having this surgery we’ve struggled to get my TSH down. I was put on 150mcg of levothyroxine from the morning after the op, and my TSH was measured around a month later to 3.26, which is far too high of course. The T4 was 16.8.

I then went up to 175mcg and remained on this until my TSH was measured in late June. In this case my TSH had reduced to 0.7, still too high but going in the right direction. The T4 had fallen to 16.1.

Went up to 200mcg since 30th June and had my TSH taken again yesterday, now finally suppressed at 0.05, but my T4 has climbed to 28.7 and is well above the healthy range.

I should note that I have some slight symptoms of hyperactive thyroid but that they’re very tolerable. For example I have a muscle twitch on my left arm, my body seems a bit warmer than usual and I’m taking slightly longer than usual to fall asleep. All unpleasant, but totally worth it to get the TSH suppressed.

My GP has arranged an emergency phone appointment for tomorrow and this has set the alarm bells ringing. I’m worried he’s going to reduce my dosage meaning the TSH will go back up (it’s been proven before that 175 doesn’t work for me, I need 200) but I do understand that it’s a game of balancing the risks and that whilst the symptoms are tolerable for now, it’s not ideal to have a T4 approaching 30.

Has anyone ever been slightly over suppressed and been able to reduce their levothyroxine slightly without a massive rebound in the TSH? I understand I have a slight budget to play with as 0.05 may be a bit too low but I don’t want it back to 0.7.

Only thing I can think of is maybe to do 200 four days a week and 175 three days a week but even at that I’m concerned my TSH will go back up beyond the 0.1 threshold

    I can't help with your question but noticed that your post hadn't had any replies yet. 

    I'm hoping that now you have had a telephone consultation with your GP you have your medication sorted but replying to you will 'bump' your post back to the top of the discussion list in case there's someone with the experience you need who hasn't seen it yet.

    GP confirmed he wants to reduce my suppression to 200 4 days a week and 175 on the other 3 days as my T4 is too high 

    I am hesistant to do this given how relieved I am to finally get the TSH suppressed, so I’m in the process of contacting my MacMillan nurse to organise a chat with the endocrinologists 

  • I think you're doing the right thing  in arranging a chat with your endocrinologist as, whilst I'm sure the GP is perfectly capable, I think when anything 'specialised' comes along then speaking to an 'expert' is a must.
