Thank you think i worked  it out ? Looks like a balloon  in side pump thet deflates slowly  and push fluid through 

Hi Nicnac. Its Automatic. I hang mine up on a clothes horse when at home just to help it when I lie on my couch,  however it’s not required. The balloon slowly deflates over the hours and you do notice it. It’s a remarkable bit of kit to be honest.
