Flap after BCG removal

After several attempts to remove all of an infiltrating BCG on the side of my nose, spreading onto cheek - which have included a smallish skin graft - it was agreed that I should have further surgery as not all of it had been removed according to the histology report.  I therefore had the graft and more BCG removed and was given a flap, with the assurance I would only be left with a dent in my cheek and obviously some scarring which would gradually fade.  Six weeks on from this surgery I now have an extremely protruding lump across my cheek to the side of my nose - much larger than the BCG which was removed. The scarring is becoming less obvious, but the lump, which I call ‘the slug’ because of its shape, shows no signs of decreasing in size and is pressing on my sinuses causing discomfort. Also my mouth is now lopsided and I dribble when I eat!

The specialist nurse says the lump is composed of fat and is unlikely to shrink by itself so will require further surgery to open up and remove the fat. I will apparently have to wait up to 6 months before this can be done to allow things to settle down.  Feel very depressed as it looks awful. I know this may be perceived as vanity and there are far worse cancers, but find it hard to live with.

Has anyone else experienced this?  And what was the outcome?

    Hello Mynose

So sorry to hear what you’ve been going through. I’ve had a different experience but reading stories on here, wounds from procedures can heal quickly and better than we sometimes expect. I hope that’s the case for you.

    Take care, Rebecca x

Question, have others experienced itching at the wound site? I have this around my facial flap and it is driving me crazy!

    Hello Mynose

Im no medical expert but I think this is a sign that your wound is healing. When I had mine on my left check I didn’t touch it but would pull some funny faces to try and relieve the itching!

    Hi Mynose,

I got some itching relief from spritzing my wound with an antibacterial spray, I picked up at the chemists. Cooling and lessened the itch a bit.

Like Rebecca says, probably just a good healing sign but watch for any unusual warmth around the wound that could indicate some infection.

Wishing you a speedy recovery & give yourself time to accept the new you. x

I’ve been reading all the posts on facial BCCs, infiltrating and micronodular in my case, but have yet to see anyone who has had a flap created on the cheek to cover the wound left from the removal of the BCC which had spread right up the side of my nose nearly to my eye.  The flap was decided upon following a skin graft to the site when it was discovered that all the BCC hadn’t been removed, so the skin graft was removed, more tissue taken and the bulbous flap created.

Please, if you have had such a flap, do reply to this as I feel very alone.

I had four head flap surgery September 26.

I had the second stage October 14.

i’ve had two post op appointments with my plastic surgeon.

The four head is healing nicely.

my nose which is 2/3 of the right side wall is swollen

bulky and red. no infection... i’ve been told to be patient.

I return December 18. I believe I need it re-opened and the fat removed it looks awful.

i’m hoping three months after the initial surgery that the skin graph is mature enough to help flatten it and take out the bulky lumpy look. I’m honestly not sure why the plastic surgeon would do it this way
