Greeting to all,
I am new to this forum and feeling a bit anxious about the possibility of having prostate cancer. Nothing is confirmed at the moment but I have a biopsy booked in two days time.
I am 48 years old with symptoms of frequent urination starting over 3.5 years ago. After numerous DRE, Cystoscopy and PSA tests I was told that my prostate was enlarged and was prescribed Tamsulosin. Unfortunately that didn't put my mind at ease so in June 2020 I did have a private MRI without contrast and the conclusion was: Prostate size 33cc. Minimal prostatic hypertrophy but no evidence of aggressive prostate cancer. Mild non-specific changes in the peripheral zone may be due to inflammation. Likert score is 2.
Last month I was referred to the 2 weeks prostate cancer pathway by my GP because of the combination of the following:
- I complained of pain in the genital area
- The urine stream was all over the place and coming out an an angle.
- My PSA (even though is low) doubled between Dec 2020 and June 2021 (from 0.4 to 0.8) but strangely enough it my last test few weeks ago it went down to 0.41 again
- GP found traces of blood in my urine
- Prescribed Ciprofloxacine
I also had an MRI with contrast about 12 days ago, the radiologist report noted the following in his report:
- The prostate measures 15 ml
- Moderate T2 heterogeneity throughout the peripheral zone
- No dominant area of abnormal low T2 signal, restricted diffusion or early enhancement. No extracapsular disease
- Normal seminal vesicles. No pelvic adenopathy. No aggressive bone lesion
- Unremarkable bladder and distal colon
Opinion: Intermediate change throughout the prostate (3/5). No dominant lesion. It would be helpful to compare against external imaging
MRi findings were graded at Pirads 3.
So, MRI grading went from 2 to 3 in a matter of 16 months. Should I be worried about this? Also I am unable to conclude if my prostate has grown in size since my last MRI because one report states the measure in cc and the other in ml. It's very confusing to me.
Sorry for the long post but I am new to this and just trying to make sense of all this information. I would like to hear other people's views especially from those who were diagnosed with prostate cancer despite having low PSA levels.

Nothing strange there: PSA is a notoriously unreliable marker, and ca be influenced by many things, including your behaviour in the preceding days. That's why it's never been adopted as a screening test: it would simply create a lot of unnecessary anxiety, while still missing some positives.
This close to having the biopsy, my advice would be to wait for the result of that, which should be 10-14 days after the test, I think.
It's certainly possible, if unusual, in a 48 year old, but the evidence so far suggests you probably don't have PCa, so I cannot help but wonder why you seem so sure that you do.
Are you an anxious person by nature, or is this a specific thing?

Think I would agree with Heinous. Probably not cancer.
Your PSa is incredibly low, I know that PSa doesn't tell all but u have more than one test result.
Pirads 3 doesn't necessarily indicate cancer .
Probably something else so wait for further tests and results.
Obviously we could be wrong but even if cancer chances are a non aggressive type.

The logical part of my brain agrees with you both but I can't help worrying about it. My wife is very supportive and positive but she is a worrying type so I know it's hard for her. I've been very busy with work in the last few days which is a welcomed distractions. My anxiety is not about the medical procedure itself as I can tolerate a lot but has to do with the possibility of having cancer and all the rest that comes with it.
Anyway, I had my prostate biopsy earlier today, it was a bit uncomfortable but nothing major to be honest. The consultant was very helpful and understanding. When I asked him if MRI findings could be a result of prostate enlargement his response was "But, your prostate size looks normal" and even said that because my prostate is "small" taking 24 samples mean that chances of missing cancer if It's there is very low. I find this very confusing because so far after having two ultrasounds, one MRI and three DRE's I've been told that my prostate is enlarged.
Like I said in my original post, the size of my prostate was reported in different units, 33cc in my last year's MRI and 15ml in my recent one. Does anyone know how do these compare? I know I should have asked the consultant but I realised my mistake after I left the hospital. He also said that he'd be surprised if I had cancer but also it was a good idea to do the biopsy in case I do. I hope he is right but I will find out in about a week's time.

It took me a couple of weeks to come to terms with having Prostate Cancer. My MRI came back as 3 (borderline). Then after the Biopsy, I had 9/29 cancerous.
My Prostate was relatively small, ended up a T2 (Gleason 3+4 intermediate risk).
Started on Hormones and am now on my 16/20 VMAT Radiation sessions.
My partner has been a great help. I don't know where I would be without her.
Before, I found it hard to admit that there was anything wrong with me, but now it is so easy to talk about anything.
The help and support from the Online Community is great. Everyone has the same thing in common, which makes it easier.
"You have been diagnosed with Cancer" are the words I never thought I would be told.
I came to terms with it, and if I can get anyone to get tested for Prostate Cancer...
