Post-RALP incontinence

A bit of background...

In September 2024, following a single episode of haematuria I was diagnosed with bladder CIS and localised prostate cancer (Gleason 3+4 = 7). The most sensible treatment options were to go on to triptorelin to keep the prostate cancer in check while the bladder CIS was treated with BCG immunotherapy. In February a rigid cystoscopy and biopsies revealed no trace of bladder cancer, so three weeks ago I had a RALP. This was apparently successful, with clear margins, although I haven't had the official follow-up with the surgeon yet.

Six days ago I attended for catheter removal. Despite drinking the provided water, I was unable to pee as everything was just passing straight through and an ultrasound showed my bladder was empty, much to the surprise of the TWOC nurse. The current situation is that I appear so be nearly continent while in bed or while slumped in a sofa, but as soon as I stand up I have no control. This is despite doing pelvic floor exercises before and after the op. While seated (i.e. slumped in the sofa), I am able to resist bladder urges.

I know that it's very early days yet, but I'm curious to know if my experience is common. Should I just be patient and wait for it to resolve? It's quite tiresome, and I can't spend the rest of my life in bed/on the sofa.

  • I am not speaking for myself, but as a result of many discussions with a good friend who was in the same position as you last June.

    His position improved gradually over the months since, to the point where he now considers himself fully recovered. It seemed to me that it was a slower journey than he had been prepared for, but effective over time.

  • Hi. My story is very similar. I am 6 weeks post op and 4 weeks post catheter removal. The first two weeks for me surpassed all expectations, dry at night and only 2/3 pads during the day. Changing the pad not because it was heavily soiled but because I was worried I was going to “smell”. Then for what ever reason I appear to have gone back in style. I went to the football yesterday and changed my pad x 3:in 90 minutes, and they needed changing. I believe my issues are around my exploration of managing loss of libido and impotence is the cause. Being a very sexual person before the op I was distressed at the thought of impotence. I was right to be concerned, I’m as dead as a dodo down there. Advised by surgeon to begin stimulating my penis as soon as I felt able to to. Week four, no sex for 4 whole weeks, unheard of so time to begin penis rehab???? I have not been given any medication for ED  at the moment so began gently stimulation, no effect what so ever. I then tried the vibrator to the glans and frenulum. Wow, and I mean wow. No erection, no ejaculation but the most amazing climax. It took me totally by surprise. Only problem is that all the good work done with pelvic floor exercises before and after has seemingly disappeared.

    Did I start too soon or is my increased incontinence a coincidence. Without a prostate the climax is I believe is based on pelvic floor contractions so could this have the cause of my increased continence issue.

    Grateful for any advice.

  • Thanks Steve.  Appreciated. Looks like I'm likely in for a long haul...

    This may be a problem, as I've only had the induction course of BCG treatment, If I'm unable to hold the BCG it renders the maintenance courses somewhat problematic. There's a 30%--70%  chance of the bladder cancer recurring, so I really don't want to miss out on the BCG. If it does return, the the only option is a cystectomy.
