To be honest ( I’m not medical!) but I don’t understand that you have aT2 with lymph node involvement . Usually T2 is cancer confined to the prostrate and no where else?? I understand your confusion .

T2 because the main tumor is contained, is asked the urologist, it spread through blood or lymph. 

Usually T3 in lymph nodes . What’s your PSA? 

Now as reference to hormone therapy beginning is 270 .

You are correct that it’s not usual to be diagnosed with T2CN1 but it’s possible and it’s like low PSA = no mets or high PSA = mets. There is always the not so usual cases.

Yes, T3 and T4 is more likely to have lymph node involvement. But it’s not a rule so, yes T2 can have N1 involvement.

Its completely random ,I already saw people with psa 300 and still contained and people with psa 20 and widespread disease .
