Continence issues

My husband had a radical prospecting and radiotherapy in 2014. Since then he has done well and remained continent.  His PSA started to rise slightly 4 years ago and he is now of hormone injections and a well controlled PSA of 0.04.  Just before Christmas he suddenly couldn’t pee and was diagnosed with and infection, and had a bladder wash to remove blood clots. His pee is now clear and his MRI showed nothing remarkable. His cystoscopy showed scarring which they think may be chasing his issue now. 
He now has an issue with peeing in the morning and has had retention which had involved an A&E visit and being sent home with a self catheterisation kit.  However once he is peeing he is unable to control it at all.  Basically he is either in retention or incontinent. 
It seems so odd as he has had 8 years with no issues at all.  Any others out there with similar experience?
