Hello and welcome to this group.
Myeloma presents in numerous ways, and the combination of features and blood measurements determines clinical stage. For this reason your question really needs to be answered by your brother's doctor.
His blood test results are not presented in the same way used by NHS hospitals, so UK patients may have difficulties interpreting them in any useful way for you.
Your brother's kappa free light chains are clearly much higher than they would be if he was disease free. The ratio between kappa and lambda light chains indicates the disease, however I know people who have had far higher levels of light chains, as well as people who have been treated with lower ones
In myeloma symptoms of damage are also relevant, elevated calcium level, renal failure, anaemia, lytic lesions (bone damage), infections, weight loss etc.
Some myeloma patients need to be treated with fairly low levels of paraprotein in their blood, or light chains, or myeloma cells in their bone marrow because of symptoms such as plasmacytomas (growths) or bone fractures/damage whereas others have no obvious problems but have over 60% myeloma cells in their bone marrow.
I hope your brother responds well to treatment, as the majority of us do.
Jane
