Hello everyone
Early this year I had a diagnosis of liver metastases.
Underwent GemCarbo chemotherapy, liver tumours had shrunk enough for a liver resection.
However, the resection operation was interrupted as portal lymph nodes involvement seen. These lymph nodes were not detected on pre-op PET-CT scans.
My questions are:
1. has anyone had local treatment like Ablation or SBRT or SIRT for their liver and lymph nodes metastases?
2. what systemic or any other treatments have people had for their liver and lymph nodes metastases and has it helped?

Another poster has brought this trail to my attention which I have never heard of before but something you might like to look into .
My mum had two liver resections . One done at our local university teaching hospital and the second at a centre from excellence for the liver . If you have not been seen at one of them it is worthwhile asking for a second opinion from them as they can be more extensive in the type of surgery where appropriate. They do the liver transplants so easy to ensure you have been seen by them . Getting a second opinion from a specialist in these centres can be quite informative even if it does not lead to the actual surgery they generally have a good feel for who is able to what in their chosen field . Sometimes it can open up other doors .
My own mum had liver resections twice but did have recurrences and set backs resulting in more systemic chemo before further surgery . I take it you got into surgery when they discovered the lymph node involvement .
I am inclined to use pubmed search engine and as long as you remember to add U.K. you can read how others are managing particular aspects like lymph nodes . Although I did this for my mum any time we want to check as they always contain prognostic factors which she had no desire to read . But it can be helpful to see who is achieving what .
Radio frequency ablation seems to be the most commonly used option in bowel cancer with a spread to the liver if they decide against surgery for some clinical reason . People seem to report to finding it very tolerable , quicker recovery and speak favourably of it .
However I don’t remember it being referred to for lymph node involvement but that could be as most go for systemic treatment .
I have always assumed they stripped some of the lymph nodes around the liver area out as they removed 73% of her liver but they never offered that information nor did I ask at that point . She had a lung met removed a few years later so I am assuming something was involved !

Yes, the liver resection op started and then they discovered the liver node mass growing around the gall bladder and pushing into the duodenum. Decision was to remove the node mass (now confirmed as metastases) but not the liver metastases. Reason being there was no benefit for me to continue with major trauma of removing 3 lobes of my liver, better to consider systemic treatment.
Please may I ask which University teaching hospital and which Centre of liver excellence your mum got her treatments?

We are in Scotland and the teaching hospital was the old Western which is now closed . However our centre for excellence for the liver is through in Edinburgh. It was excellent. Lived up to its name .

I am currently recovering from the liver surgery. Plan is to start Nivolumab immunotherapy in about a fortnight.
All my chemo, radiotherapy, more chemo, failed liver resection and now immunotherapy is at The Royal Surrey Hospital.

That’s a great plan . My friend had an amazing response to immunotherapy . Hoping you do too . It’s great to see it coming through on the NHS . I have been watching the success in America .
