My husband was diagnosed with stage 4 clear cell renal cell carcinoma with lung mets back in early March. He was quite poorly at that time, very anaemic, no energy, huge weight loss, literally wasting away before my eyes. We were told from the outset that his condition was incurable but treatable, with the treatment aiming to improve his symptoms and improve his long term survival prognosis.
He has been on Avelumab and Axitinib for 7 weeks and what a difference it has made. His cough has pretty much gone, he has gained half a stone and his appetite is much improved, he is stronger and happier in himself. There have been side effects of this treatment, mainly skin problems, itchiness, a rash which is mainly on his scalp making it difficult to apply creams etc, and more recently in the last week, sore and achy joints, particularly his knees, a couple of episodes of diarrhoea but nothing too bad. He will stay on this treatment until it stops working or the side effects get unmanageable.
What are other people experiences of this combination of drugs. We are only just at the start but it seems like he has responded very positively to it. At what point should he get a ct scan to assess if the tumour has shrunk and by how much. How long do people generally stay on this combination, what do we need to look out for, we know about the potential for high blood pressure but no problems so far with this.
These are all questions we need to ask his consultant I guess.
He has a telephone consultation with either his consultant or dedicated cancer nurse a couple of days after each infusion but since starting the treatment he has not had a face to face appointment, although he does see nurses and doctors when he goes for his infusions once a fortnight.
I am also a bit confused about his level of immunity to viral and bacterial infections in general. We were given a warning about sepsis right at the start but, is he classed as immunosuppessed on this treatment, should he be avoiding contact with other people??
As he feels better we are wanting to go out to places more but I worry about him catching something.
If anyone has any advice or info which we may find helpful it would be good to hear.
Anyway, on the whole we are feeling much more positive, long may it continue.

Unfortunately I cant answer them, being on Pazopanib, not any kind of immunotheraphy/ combo but I hope someone else in the group can and will.
Yes, you definitely need to ask his consultant/ specialist nurse these questions. Make a not of them all, - plus the answers when you get them,so you can refer to them later.
I have regular 3 monthly CT scans so I would say your husband needs them at a similar interval

Hello, my father (77 years old) was diagnosed too in March with a stage 4 clear cell renal carcinoma (diam 10x13x10 cm external of the left kidney) with lungs mets of less 2 mm diameter, poor outlook. In Italy we have as first line treatment with the public healthcare the combo AXITINIB + PEMBROLIZUMAB, and he started with it at the end of April. The only side effect is high blood pressure, which peaked to 187 max - 107 min at the end of May, the day of the second dose of Pfizer Vaccine (ANTICOVID). So basically he had to discontinue the Axitinib for a week and the blood pressure got back to normal value. Monday got the second dose of immuno and he started again the cure, the blood pressure is stabled around 140 - 90, taking also Ramipril 5mg 2x day. In my opinion more time pass and more I see him weaker and skinnier, I don't know if the cure is good, anyway to answer your questions in Italy my dad get ct scan after 3 immuno infusion, 1 immuno infusion every 3 week. The blood tests improved after start of the cure, the platelest went back in a normal range and also the anemia improved. I read a lot of academic article on the axitinib + pembro combo, the progression free survival median was of around 14 month if I am not wrong. The cabo combo could be the best for PFS of more than 20 month but the Overall Survival didnt improve against sutinib. I don't know much about your combo tho. If I can help you more feel free to ask.
