Break through pain

Hi everyone,

Just had my weekly review with Dr , and she is very happy with my weight ,only lost just  over 1 kg so far , been on tube feeds for the last week. 

Pain has ramped up considerably, so I asked for more relief ,she's quite confident that I don't need morphine patches at moment ,but has upped my pregabalin and gave me oxcodone liquid for break through pain ,which I really do need now as pain is a constant. She will revise my meds again on Monday to see how I got on. 

I have dietician coming tomorrow as I'm starting to use a pump for feed ,and she needs to show me how to set it up. I can't fit 6 bolus feeds in in a day without feeling sick. 

And I do need to up my Laxido!! 

Anyone else been on a similar regime ?

  • Hi. I think there were quite a few of us on opioid patches which are fentanyl. I’m sure  was

    I had morphine. Fentanyl I think is more powerful.

    Pump feeding overnight is the way to go, frees up the day for other activities, bit of a pain sleeping with the set-up....my wife scarpered to the spare bedroom . ..make sure you have spare Y-junctions...they can become brittle with use.

    Laxido and morphine is fun, 3 days bunged up then 3 days turbocharged...a science lesson.

  • Yes Peter ,it's going to be a learning curve with the laxido !! 

  • Also I'm told the tubes are single use on my pump so no cleaning !

  • PEG I used had a Y junction at the feed end which was prone to breaking, maybe this has been successfully modified

    I took one night and morning. You soon know if you are taking too much. Too little is much much worse by all accounts.

  • gave me oxcodone liquid for break through pain ,which I really do need now as pain is a constant.

    Don’t let your doctor make this decision for you if your pain isn’t controlled. Everybody’s pain perception and tolerance is unique. Constant pain isn’t breakthrough pain. Breakthrough pain is when your background analgesia isn’t enough and you are getting peaks of pain when your short acting stuff wears off unless you do something about it. 
    I was on four hourly oromorph ( which has around the same duration of action as oxycodone unless you are on the long acting prep) I was told to report if pain was waking me in the night. When it did I had long acting morphine for background pain and oramorph through the day. It was a game changer

  • Hi I had co codomol and oramorph ( liquid) and I fed during day which I found easier than night feeding. You’ll find a regime that suits you. Yes keep on top of laxido it’s vital that you don’t get impacted poo.

  • I was on Oxycodone in liquid form and towards the end on Buprenorphine 10 microgram patches.  I think the various types of pathches are probably similar in their potency and effect - just work differently and it may be that one is better suited to your type of pain than the other.  I guess it may take some experimenting to get the right medicine and dose.

    In the end I was on something like a 65mg morphine equivalent per day.  Both Oxycodone and the patches are significantly more potent than morphine.

    In any case they do dull the pain but certainly don't eliminate it.

    I was on both the patches and Oxycodone for some weeks post treatment.  Make sure your GP/hospital team are really careful as to how you come off the opioids as withdrawal can be a big issue, but it is the price to pay for the pain relief.
