Morphine not working for pain relief

Hi all

I'm posting on behalf of my mother who is 3 weeks into her radiotherapy for tonsil and back of toungue cancer. She was supposed to have weekly chemo and daily radiation for 6 weeks but could only manage 2 chemos as they made her so ill.

At the end of week 2 of radiation her side effects have kicked in thick and fast. She has ulcers and oral thrush and says her pain is 10 out of 10 plus. She has been given cocodmal, oral morphine and 10mg slow release morphine (going up to 20mg this evening). Despite having a lot of morphine, it's only the cocodamol that seems to give her relief but she can only have this 4 times a day so a lot of her say she is in excruciating pain. 

Has anyone else had a similar experience with morphine not working and cocodamol being better? Does anyone know why this is? And finally has anyone been offered a different pain control regime that has worked well for them? She may go on Oxycodon soon. Is that any good?

She has three more weeks of this and is so ill. It's awful to see and I hope there is better pain management.

    Contact your Mum's CNS/cancer triage ASAP, out of hours number is usually on paperwork,  pain meds will need reviewed/adjusting, nobody should be suffering to your Mum's level.

    I too found that liquid morphine did not help my pain much during radiotherapy. I found 4 hourly panadol helped more and I was also prescribed Fentanyl patches. I'm afraid I never got my pain under control completely but it did help dull it so it was more manageable.

    I asked to see my radiotherapy doctor before my radiotherapy session and he was very helpful with trying to sort out the pain issue. Get in touch with your mum's radiotherapy doctor as I am sure he will be able to help.

  • I had awful pain with recurring oral thrush and ended up on fentanyl patches for a few weeks, along with the thrush treatment, cocodamol and morphine as needed. Came off it gradually over another few weeks as things got better and I had no problems with stopping it. My GP prescribed it and it was the only pain control regime that worked for me. Hope your mum feels better soon. It's a horrible time. 

  • For some people the body does not metabolise morphine very well.  I am one of those.  Morphine for me is as much use as sugar water.  I found tramadol was good as was oxycodone.  Speak to her clinical team to get something that works.  I also found paracetamol was very effective, but short lived.  Try and space out any painkillers so that they are more effective than I'd all taken at once

  • Oh that's great! I hope they work as well for her as they did for me and she gets relief from the pain. The oral thrush was definitely 10/10 pain for me too. I'm feeling very well at the moment thank you. Finding out if my treatment worked next week, so Fingers crossedFingers crossed

    So glad to hear that your mum is now on better pain relief. It does make all the difference. I am doing fine now. Have not needed pain relief for some time and have got back to enjoying my life. Best wishes to your mum.
