Post op pain

Hi everyone 

just wondering what others experiences are:

4 weeks ago I had left maxillectomy with reconstruction with forearm flap,neck dissection and tracheostomy. Im 52 yrs old prev fit and well but struggling with pain. Im on regular paracetamol and brufen and I have a bit of oramorph prior to bed. The dr isn’t keen on me having morphine and says it’s muscle pain and to just accept it but I find it quite debilitating to be honest. It’s awful and the painkillers I have aren’t enough but I think he thinks it’s all psychological?? I also have trismus and massage and do exercises but I’m fed up of it. Probably nothing compared to what many of you are facing but is this normal to be so painful or am I a wimp and need to man up so to speak (as that’s the impression my consultant gives me , although he is lovely). 

  • Oh dear. So sorry to hear that. You’re post makes me so angry. Really angry. It’s a terrible attitude to pain. It doesn’t matter what’s causing it. I don’t think your consultant is lovely at all! Pain is better controlled proactively by taking pain meds by rote. This means taking your oramorph and paracetamol by the clock. I’d contact your CNS and insist this is not acceptable.

  • sorry ER1 I didn’t mean to be so abrupt. I really feel for you. You need to be putting your energy into healing not fighting pain. It’s a really drastic surgery you’ve been through and a lot of pain is normal.  I hope somebody sorts this for you tomorrow.

  • Hi ER1, you are not a wimp and you don't need to man up, you have had surgery, and are now in pain due to this surgery, and this pain must be eased for you. As Dani has said your body needs to recover from the surgery,  not trying to fight the pain. I hope you can get some better pain relief. All the best.

  • Hi ER Never apologise for being in pain, as my oncologist put it cancer is bad enough cancer and in pain isn’t good enough. I didn’t have surgery but I had radiotherapy and chemotherapy. I took pain killers by rote ,as Dani says it’s hard enough to heal never mind trying without sufficient painkillers.I was on constant pain relief for weeks 30 mg cocodomol ibuprofen and oramorph. I took up to 40  mil of oramorph when I was at my worst. So please please contact your team tomorrow.Stamp your feet if need be,Your pain medication needs looking at paracetamol has its uses but …….

  • I’m so sorry to hear your in pain, it’s natural you’ve had surgery you are not a wimp, to recover you need pain management. Like Hazel I am angry that you were told to accept it. Speak to your team to get the correct pain medication.  You have enough to contend with recovering, without not having pain reliefI too had Trismus, and could only open my mouth wide enough to get a baby toothbrush in. Speak to your team about this as well, I was given a Therabite machine and my mouth opening has gone from 15 mm to 32mm.

  • Thanks, no problem. I had the pca taken away 2-3 days post op as the consultant doesn’t like morphine! 
    I’ve spoke with them and CNS and she said take 10mgs oramorph at night whereas he isn’t keen. However he is gonna do an MRI to check but he laughed, to prove to me all is ok! 
    just gets you down and I understand about the risk of constipation which is a big problem but still seems unfair. Next f/u in 2 weeks 

  • Thanks Ray, I agree but just keep getting told it’s muscular and normal so just have to accept it . Difficult when quite debilitating Shrug♀️

  • Morphine constipation doesn’t exist if you take movicol or laxido twice a day. I was in oramorph and long acting morphine for nine weeks and managed a comfortable loo trip every day

  • Hi Hazel, I take paracetamol and brufen, watching the clock for when they’re due and oramorph at night 

    I’ve seen them today and I’m having an mri to check all ok but consultant sure all is ok, it’s just muscular and I need to accept it so my exercises and will be like this for next 2 months or worse if I need radiotherapy-still waiting for histology results

  • Wow that’s good to hear your mouth opening has improved so much. My mouth opens 1 finger width. Other than TMJ exercises I haven’t been offered anything else and I can only eat with a teaspoon and use a baby toothbrush too, I actually don’t think I’m cleaning my teeth properly as can’t get in there :( 

    I’m back in 2 weeks I’ll ask about the therabite machine, thanks Blush
