Life insurance, declaration of serious ill health?

My dad has asked me so check in this group to see if anyone has had a similar experience and can share, 

we have a declaration of serious ill health form from my mums life insurance company, we fill out the details and the gp/medical professional has to sign this to confirm the information is accurate so we can send it back to her insurance. However, we took this to my mums gp practice and they said it’s a minimum of £100 to get this document signed, just wondering if anyone has had this done before and if this price seems accurate? 

or if anyone has had the consultant sign this document instead and did they charge? Just seems an awfully lot just for a signature. 

just looking for anyone who may have similar experiences or have used this form before.

  • I don't know what your form is for exactly, but if it's for a life insurance payout (critical illness, terminal illness) I think the life insurer generally pays the doctor's fee. But often they don't pay in advance, and the surgery don't want to do the form without prior payment. It's worth checking this out. This was our experience anyway. But it may be you're doing something different.

  • If you are talking about critical illness claim, I attached the consultant’s monthly report. I don’t remember going to GP for this. GPs do charge for such things which seems quite unrealistic. I would say call the insurance and chk with them if they can cover the cost and also chk with the onco team. 

  • We had a DS1500 form from the consultant that unlocked critical illness payments and getting my wife's pension early. I'm surprised at the £100 charge, that's the sort of thing solicitors do but I wouldn't expect it from the NHS. It's not like it's a voluntary thing like needing a travel vaccine. 

  • I just remembered our insurance directly reached out to the GP after I submitted the form with consultants report. The form had section where I had to fill in GP details.

    I think the fee is at the discretion of the GP practice. We had a similar fee mentioned when my dad needed a form signed.

    When it was G and I needed a letter for the critical illness claim, I used the discharge letters from the hospital and didn't need any further signatories. 

    We did need a letter for the pension company at one point and the oncologist provided that.

    All a bit of a red tape minefield that you don't need to be battling w.ith at this time.
