Had a real set-back last week and feeling so anxious today. We’ve been awaiting the results for some of the tests on the biopsies they took on 23 Dec, the so-called FISH tests. Well, they’ve come through now and the histology report indicates I have ‘Double-hit Lymphoma’ which is quite a rare kind or sub-set of the previously diagnosed DLBCL. All this being a transformation from my Follicular Lymphoma I was treated for over the last six months. So many types and variants! Bottom line is, this is more difficult to treat and the treatments are even more complex and intensive.
We’re going in to see my consultant and specialist nurse today, when we’ll find out more about the disease and treatments, then it’s their MDT on Wednesday. My nurse said they might propose DA-EPOCH-R or R-CODOX-M with R-IVAC, but they’ll discuss further with us, prior to consenting. My second round of R-CHOP that was due on Wednesday is now cancelled and it’s likely I’ll start on the new treatment on Monday 7th. Also, this time I’ll be staying in the hospital for at least a week for the treatment. That’s it in a nutshell. Obviously it’s not good news and a further serious development. We of course are very frightened and anxious again, as we have a lot of questions, but we do have great faith in the medical team and with their support we will beat this!
Hi Jona, so sorry to hear your news. I was diagnosed with stage 4BHX DHL in October, which started as stomach pain. I have been having DA-EPOCH-R since then. Because it's so rare and only recently understood as being a distinct type of Lymphoma, I've found much of the information online to be pretty negative about the prognosis, which is not my experience.
DA-EPOCH-R has worked astonishingly well on me. By the end of my 1st cycle, the pain had started to subside. By cycle 3, I was off the morphine and my PET-CT scan showed "complete metabolic remission" i.e. they couldn't see any active trace of it. I'm now on cycle 5 of 6. It is a tough regime with all the hospital stays and some nasty side effects, but I've never found it too much. When the side effects started getting more severe they adjusted my dose down a bit, which helped.
Because DHL has a higher risk of spreading to the central nervous system, I've also had 3 doses of Methotrexate via intrathecal. This is a pretty straightforward procedure, just a couple of hours on the day unit and no major side effects. I'll also have 2-cycles of high-dose Methotrexate via IV after my DA-EPOCH-R. All this is just precautionary, they looked for lymphoma cells in my spinal fluid and thankfully didn't find any.
I had "bulky disease" so they're considering some radiotherapy to my stomach at the end of it all, again just precautionary.
Even if the DA-EPOCH-R hadn't have worked, or if it comes back, there are further treatment options, including stem cell transplant and CAR-T therapy, so DHL is definitely beatable! Good luck today (hugs)
Great to hear from you and hear I’m not the only one! Thanks, your comments have been very helpful for me. My treatment has been confirmed and the drugs have been ordered for Monday - DA-EPOCH-R, and by the sound of it they will need a shopping trolly!
