I'm 7 days in on my second cycle of Abemaciclib - and I am struggling. I'm juggling episodes of severe diarrhoea and bowel pain with loperamide which then causes constipation. Trying combinations of drugs under advice hoping that there is a 'normal' stable situation somewhere on the horizon to be achieved. White blood cells fell below acceptable levels after hideous first cycle - had a week off it - but now back on it again. I would be pleased to hear experiences about balancing diahhorea management with Loperamide and trying to avoid the vicious cycle with constipation and if/how anyone has managed to achieve a near normal existence on this treatment. I am told it gets better as you get used to it - is that anyone's experience? I am hoping to start a phased return to work in January but worried this isn't going to be achievable. Thank you
Sorry you are going through a rough time. I found that taking 1 tablet of loperamide instead of 2 for the first dose after an episode of diarrhoea. This helped me with the constipation and bowel pain. But I don't have really severe diarrhea and often need just need the one tablet. Very occasionally I need a second the same day. On the whole it is manageable. I am on cycle 3.
I’m only on my 1st cycle but have already found that I need to take my 2nd tablet just before bed
taking 2 hours either side of a meal is disastrous… might be worth looking at your timings?

First cycle for me I couldn't leave the house unless it was the day after I'd taken loperamide - so I'd have one day of diarrhoea and one day usually not then it would be back the next day.This cycle I've tried taking regular loperamide as advised - one loperimide with my morning abemeciclib which is better but is then giving me the build up to an really bad episode every 2-3 days. Didn't take any loperamide today to relieve potential constipation and back on laxido today also which has helped. But now wondering how to proceed - maybe one loperamide in the morning every other day to try and keep it under control I'm thinking - and dose up if needed?
I'm doing 9am and 9pm for my abemeciclib. And was trying 1 loperamide with morning dose.
I take my abemaciclib at 7 and 7 as I am up around 6. I have mine after I have had my breakfast and an hour or so after my evening meal. I had a look online and the evidence seemed to suggest that timing of the tablets and if they were with food didn't make a difference to side effects. But I understand that is not everyone's experience.
I don't take loperamide regularly but only after an episode of diarrhoea. One loperamide usually gives me 2-3 days side effect free after an episode if I avoid triggers like dairy and pulses. But maybe I am lucky.
I can understand the logic of taking it as a preventative measure to stop or reduce it before it starts if the diarrhea is severe.

It's really pants at times, isn't it (literally)? I started abemaciclib back in April after all the 'big' treatments had finished, as well as the little treasure that is Letrozole (the gifts that just keep on giving...usually when you really do not need that gift).
I tried 150mg for 3mths & started to think my rear end would never be the same again. As one of the other ladies said, loperamide would sort of 'buy' me a day, maybe two when nothing (at all) would happen then just as I'd start feeling slightly human, off it would go again.
Oncologist reduced it to 100 for a couple of months but still no real difference. We had a good chat about was this really worth it, even though all my bloods were consistently OK & decided to give a final reduction to 50mg a go.
Within a week everything was fine! I'm on my 4th round on the lowest dose & only have to go for bloods at three months so 'party party'. My rear end def needs a bit more r&r but, oh so much more bearable.
