DCIS and awaiting surgery no.3

Hi, I am new to this group. I’m 38 and I was diagnosed with DCIS after my first surgery in January. Second surgery delayed as I had COVID so second surgery start of April. Results back today and margins not clear. Now it is no longer a low grade but intermediate. Not sure what this means.  They want to do a 3rd surgery and remove the nipple and go deeper with tissue removal. If that doesn’t work it’s a mastectomy. Has anyone any experience of reconstruction after nipple and deep tissue removal or whether it would be better to have a mastectomy with a reconstruction. Looking for reassurance.

    Did the surgeon say what the proposed surgery would mean for the shape of the breast? I would ask that, as well as which option would give you the better outcome both in terms of appearance and feeling.

    Going with the lumpectomy does run the risk of ending up needing a mastectomy anyway, but it may be worth it in the long run if it's the better option since it does have a good chance of succeeding or they would not have offered it. If, however, the lumpectomy would leave you with an odd shaped breast, then perhaps it's better to opt for a mastectomy now, seeing the nipple would be gone either way.

  • If you had a mastectomy you could also have an immediate reconstruction if you wanted one. I would ask questions about the surgical result and shape of your breast after further surgery if you didn't have mastectomy.
