Margin re-excision / G3 IDC & high grade DCIS

Hello, I was diagnosed with a grade 3 IDC and had a lumpectomy in May. The subsequent biopsy showed an unclear margin plus 1 out of 3 lymph nodes with cancer. When I received the histology report it also referred to an area of high grade DCIS 'within the boundaries of the invasive tumour'. Approx 5% of the IDC tumour was not removed and this is adjacent to the high grade DCIS which has resulted in the unclear margin. The pre-surgery MRI did not identify the DCIS and none of the post-surgery discussions or letters made any mention of high grade DCIS either. Following the lumpectomy I was put on a 6 month course of chemotherapy with the plan to re-excise the margin and possibly undertake a full lymph node clearance after the chemo.

The oncologist has said that the chemo is adjuvant, to mop up any stray cells and prevent a recurrence, and there is no need for any scans during chemo (despite not all of the IDC tumour & DCIS having been removed). They also recently advised, when I asked about the DCIS, that this would not respond to chemo so they had been focussing on the IDC. The surgeon has recently said that the plan is still to re-excise the margin after chemo and to now definitely have a full lymph node clearance. I asked about the removal of the high grade DCIS, and the apparent lack of information with regards to how large this is, and asked if a mastectomy would be more appropriate and was told not at the moment, but there are no guarantees that the margins would be clear so options would be discussed after the next surgery if necessary. I asked how much breast tissue would be removed - quite a lot has already been removed - but was told that they wouldn't know until during the surgery. I was also told that they won't be able to tell if the chemo has worked, although there may be some evidence of this within the removed lymph nodes and depending on how much cancer is found they may need to consider re-staging and other treatment options (CT & Bone scans 6 months ago showed no spread).

So, I'll be having the re-excision approx 9 months after the first surgery but there are no plans to have a further MRI before this to check on the size of the remaining IDC tumour and area of high grade DCIS (I will keep requesting this). I'm concerned that the high grade DCIS appears to have been overlooked during my care plan and the re-excision won't be successful, so it would be good to hear from any others who have had margin re-excisions which were successful first time, or had a re-excision on a joint area of IDC & high grade DCIS.   • Thanks for explaining. I’m pleased you are at the tail end of the chemo journey. 
    That is strange, the cynical side of me perhaps wonders if there is an element of track covering, but I hope not. 
    I haven’t had any letters since my first hosp apt in Sept, not even with my diagnosis, just heaps of appointment letters. Is that strange?

  • Hi Mickey, since my first hospital appointment in April I've been receiving letters sent to my GP (following meetings with the surgeon & oncologist) that I have been copied into but these have usually been arriving around 2 or 3 months after they were written. I raised this with my nurse last week and it's apparently a common problem which they have been trying to improve. I'm surprised that I received the letter today so soon after the appointment but it may be because I raised the problem. You might find that your letters have been delayed too - might be worth asking your nurse if they can send you copies of everything that has been sent to your GP? Mine has been good at emailing me test results that I haven't been provided with, and it was by looking at all the letters sent to the GP that I could identify various discrepancies between them regarding grade, sizing, margins, and the lack of any mention of the high grade DCIS, also discrepancies within the histology report, and also between what I had been told at meetings and what had been said in the GP letters. One letter even referred to the tumour being in the wrong breast! Good luck with your treatment, hope everything goes well for you x 

  • Thanks for this. If my breast care nurse ever rings me back, lol (I was promised a call Monday), I will ask, everyone seems to know their scores etc and actually. I only know the grades and types and that I’m oestrogen positive and HER2 negative, but nothing in writing.

    Hi MickeyM based on my own experience getting the scan is positive and once this is done you should receive your surgery date. I found out that the hospital probably has a provisional date but will not confirm until the scan is done.

  • Thanks again, when I asked last Fri, I was told a provisional date would be booked within 5-7 days. I’m just anxious to know it with Christmas round the corner also x 

  • Hi, just to update, I had a bit of luck today as I was at the hospital to see Plastics BCN, I popped with my letter to the CT scan area as I had a genuine query and they agreed to do my CT scan after my apt. A bit woozy, what do they put in the cannula that makes you think You need a wee? Lol!! Also I have a provisional date subject to CT results confirming blood vessels ;or whatever) ok and no higher priority if 14/12. Hallelujah and fingers crossed - Mickey x

    Hi MickeyM this is great news for you as you can now make a plan.The scan is a bit weird but you have had it done now and the results will not take long to be communicated to the surgeons. Looks as though you will be home for Christmas although will not be able to do much this year. Fingers crossed for you x

  • Thank you Queen Poppy, I really appreciate your reply. My BCN says they will give me a front fastening bra on the day of the op and the flexifit zip ones from M&S she says would be fine, so I’ve bought some cotton button front nighties/dresses also, which she said would be perfect. Fingers crossed it goes ahead as planned now. I’d already pretty much cancelled Christmas anyway, this is much more important to me to get done. 5-7 days in hospital they said.     Hi MickeyM I am so pleased you have this sorted. If you can stay in 5 to 7 days that would be good as I felt going home after 4 was a bit too soon. If you have drains you will have a better chance of going home without them. Saying that mine in the breast was removed early but I was lucky my surgeon used a procedure that did not use drains in the abdomen. You will get through this and let me know how it goes x
