Help with decisions

Meeting my surgeon on Monday to see if after four local excisions my margins are now clear- I had DCIS/DCIS-Mi

if clear (so hoping,) then it’s over to clinical oncology for radiotherapy and it’s likely they’ll want to start me on hormone therapy. I’m very nervous about hormone therapy because I had a major stroke caused by The Pill a number of years ago.

I am perimenopausal too.

if the margins aren’t clear, then they may recommend a 5th excision or mastectomy. I’m getting to the stage of just going for the mastectomy, because I am so fed up.

So many things to think through- my head is all over the shop.

Re hormone therapy, would it be worth asking to have a meeting with an oncologist and a stroke physician, to help come to a decision? Ideally a face to face meeting.

Re potential need for another op, does anyone have any advice as to what I should be asking my surgeon. Wondering if it might be worth asking to speak to a female surgeon to get another opinion?

I have private health insurance, so could get another opinion. But I don’t know….

Any thoughts would be very much appreciated.

  • Hi  , no wonder your head is all over the shop, that’s a lot going on. The dilemma re further attempt at clear margins versus mastectomy is really difficult and comes up quite a lot here. In your situation I feel like I’d be tempted to opt for a mastectomy if it’s on offer, as you’d hopefully know for sure that there’d be no more surgery. But I haven’t been in that situation so it’s difficult to know. If you do have a mastectomy, would that mean you wouldn’t need radiotherapy? I’d definitely ask about that. Re the hormone therapy, they should discuss any existing health conditions with you at your oncology appointment. Sometimes there might be an alternative drug that may suit your situation better. For example, I’m on Anastrozole because there was no doubt that I was post menopausal when I was diagnosed. But an older friend is on tamoxifen, because she already had osteoporosis so the bone thinning aspect of Anastrozole or letrozole were deemed too risky. But you could definitely ask for a discussion with a stroke specialist too, seems like a good idea and the worst they can say is no! And I guess that would then be the time to use the private insurance? 
    Good luck with it all, I do remember feeling at one point as if my head might explode as it was so full of ifs and buts! It’s horrible. Love and hugs, HFxx

  • Thank you so much Happy Feet, such a helpful answer, appreciate it. Yes, I trust the system, so believe they will be bringing in the experts for advice in relation to my stroke. Will certainly ask the question. Keeping my fingers crossed for no more surgery meanwhile.
