Difficulty with sex

Hi - I’m on abemaciclib (almost done) plus letrozole and zoladex after hormone positive invasive breast cancer with lymph nodes affected as well 2 years ago. My periods stopped on chemo and haven’t come back so I guess I’m in a medical menopause aged 46. I’ve found penetrative sex with my husband impossible - it’s really uncomfortable so we resort to other things - but I was wondering if anyone’s experienced this and found anything that works. I know I can’t have any oestrogen related treatments. Thanks in advance for any suggestions. 

  • Hi Runandbecome welcome to the forum . You raise a very valid point and one that people find difficult to ask about but it affects us in different ways. You have come to the right place to discuss this and we all know what the issues are and can be. I had real issue with Vaginal dryness and still do to a huge extent but I used Hyalo-femme that you can get on prescription. Its  a vaginal gel and ask for an applicator otherwise its a challenge to insert. There are many others that are not hormone based and it is a bit of trial and error until you find the one that suits you best.

  •  Hi, I use Replens, which I get on prescription and it helps with day to day discomfort due to dryness. There are many lubricant gels which we find useful to make sex less painful, but I hear you, it's still uncomfortable.  Best wishes

  • The zoladex and letrozole will be keeping you in a medical menopause and the combination means pretty much no oestrogen is produced by you at all.

    I am similar and found the vaginal atrophy I had after chemo and then the continued hormone therapy has been pretty bad.

    I had replens from my GP which was ok but I preferred Activ Moisture, which you have to buy. I now use Naydaya Victory oil, which is really good for moisture and comfort.

    I use Durex Sensilube as a lubrication during sex, which we have found has the most natural feel.

    The sex is more difficult because I am just so atrophied that everything feels like is has closed up!! It was so bad at first that I ended up getting some vaginal dilators - those at least got me to a better state, but I still have that tightness which makes penetrative sex uncomfortable at first. 

    I loved sex so this has been a bit of a grieving process to accept the new way of things.

    I'm still on hormone therapy so not sure if things will improve a bit once I stop that.

  • Hi  I’ve had similar treatment to you, finished Abemaciclib in Feb this year - I didn’t have Zoladex though as I was post menopausal at diagnosis. 

    There’s an organisation called Menopause and Cancer which (as the name suggests) is all about supporting women through menopause after they’ve had cancer treatment and don’t have the option of mainstream medication. If you go through the podcast library, there are quite a few on vaginal health and sex which I’ve found really helpful. I think it’s episode 72 that had a representative from Yes which frankly, had a game changer piece of advice for me!
