Lumpectomy or Therapeutic Mammoplasty?

I'm 68 and due to have surgery . My surgeon hopes to do a therapeutic mammoplasty. I'm wondering if a lumpectomy will be more straight forward. My healthy boob might need reducing at a later date to match the mammoplasty.

My tumour is  below my nipple towards the centre of my chest ....surgeon hopes nipple blood supply is unaffected. Might do lumpectomy if scans on the morning of my op suggest this. He can't feel my lump and I need wires to mark the place.

Does anyone have experience of a similar scenario?

  • I had wire guided lumpectomy but area removed was from top of breast. The wire insertion sounds horrific but it really isn't it.  I had it inserted while they did mammogram and they gave me a local anaesthetic so didn't feel it.  It's very fine wire and they coil and cover it so you don't know it's there.  It's removed during surgery.  I was initially offered a reduction at same time on unaffected breast but after their MDT meeting they decided it wasn't necessary.  6 years on and only a slight dent in breast and my breast only mildly different in size.

  • I was given a choice and after several chats with surgeon and lots of research, I went for a therapeutic mammoplasty. This was in August 2021.  I'm happy to talk more about it if you want to message me.  I'm 63 by the way.

Agree with the wire is nothing to worry about it.  It's not painful and is really useful for the surgeon to find exactly the right area.

  • Hi

I had a 4cm dcis removed in December. The dcis was in the upper inner quadrant on my right breast.Due to size of the dcis at the mdt review, a mastectomy was proposed. However when I met the surgeon she thought mammoplasty was possible. I'm a 38 c/d.

Given the size and position  of the dcis I was expecting a reduction of two cup sizes.

I'm 63 and really pleased with the result of the mammoplasty. It resulted in a breast lift so my right nipple is slightly higher than the left but regarding cup size and appearance there isn't a great difference and in a bra its not noticeable at all. I also had 4 sentinel nodes removed so have a scar under my arm.

Given the size and position, a lumpectomy wasnt really an option for me. The operation was straight forward and after 4 weeks I'm recovering well.

Feel free to ask me any questions if I can help xx

  • I had this option though my lump was in the upper quadrant near my armpit and I had chemo first so the lump had all but disappeared by surgery.  I opted for the lumpectomy, and whilst the mammoplasty would have been great in giving me smaller boobs (am a G) I wanted the least invasive surgery and didn’t want the risk of infection from multiple wounds (he would have done the other boob reduction at the same time) to delay radiotherapy.
