TNT for stage 3 rectal cancer

Hello everyone 

I have recently been diagnosed with rectal cancer T2N2M0.

So my treatment plan is TNT starting with 5 days of radiotherapy followed by 4 rounds of Capox chemotherapy.  As my actual tumour is only small I think they are hoping that I can achieve a watch and wait situation after my chemo, but I do have some extensive lymphnodes involved. 

Has anyone else had TNT treatment with short course radiotherapy?

Did you achieve a watch and wait situation and are you still disease free and for how long?

  • I’m on TNT, diagnosed with stage 3 EMVI+, rectal cancer T3N1M0, back in May. I had Long course chemoradiotherapy, 5 weeks 23 Jul - 26 Aug and then started 1st of 4 cycles of chemotherapy CAPOX 22 Sep, 3rd cycle begins Monday. I am due to get my first scans post diagnosis on the 10 November, which will give the first indication of how the treatment has impacted the tumour. My consultant has suggested the TNT approach doubles the odds of achieving a complete response and with it go on the watch and wait. Apologies I can’t give you the full answer to the question set, but hopefully it will give you a little more insight into the process. 

  • Thanks for replying. I notice a lot of people are having long course of radiotherapy or chemoradiotherapy.  I am hoping mine is short due to my tumour being so small.  My week of radiotherapy starting on 10th November.

  • From previous posters it appears the size of tumour has little impact, depth potentially being more important, my tumour is 4.5cm, but depth indication can be taken from the T3 diagnosis, suggesting your T2 tumour has not grown as far into the bowl lining. Both treatments I have had so far been doable, not pleasant, but starting treatment for me at least was a relief, as it starts the journey to recovery. Best of luck with treatment.
