My partner’s cancer journey started in August 2024 when he was diagnosed with colorectal cancer stage 3 - lymph nodules involved. He has had 4 cycles of Xeloda which was hard work due to his dpd deficiency. He had successful operation in March 2025, primary tumour removed with lymph nodules and no stoma bag.
6 weeks post operation he had CT scan which showed very small dots on his liver which was later confirmed by MRI that cancer has spread to the liver and now he has 30 very small liver mets which equals to 30% of his liver engaged by the cancer but it’s across all 8 segments, so unoperatable.
He has had 5 cycles of combined chemo: 5FU, Irinotecan and Cetuximab, rescan after cycle 4 which showed that liver mets have grown and cancer is chemoresistance. He is now on Lonsurf and Bev/Avastin. Whereever we read it’s not promising and it’s only given to him to follow NHS protocol.
We have had private consultations and they suggested SIRT as targeted radiotherapy, has anyone here had it done? Is it even worth doing? Has it make your liver mets suitable for ablation? As it may extend his life by 3-4 months.
We think NHS oncologists basically know he is going to die and just tick the boxes. He is 51 years old, fit, still doing his exercise.
At get-go they told us chemo for liver mets and maybe liver transplant but he would have to be stable for 2 years, do people even last that long on Lonsurf and Avastin?

Hi, although I don't know that much about SIRT, (another thing I'm not allowed to have) I have been on Lonsurf and Avastin for 6 months. On and off. It seems when I'm actually allowed to have it, the scan has showed stability or shrinkage of liver mets. The trick is having a good oncologist that allows you to stay on it. Neutrophil levels (white blood count or something) goes low with most people on this treatment. If that delays your treatment, I've found getting the blood test done on the day of treatment is best. My levels have been 1.4 on Tuesday and back up to 2.3 by Thursday (my usual chemo day) there's a lot of people surviving a long time on Lonsurf and Avastin. There's also 4th line treatment available if you see progression on Lonsurf. Fruquintanab and another one too. Frustratingly I'm having to fight the NHS to get me on them as my oncologist has seen one scan that's showed a tiny bit of progression and pulled the plug on my treatment. But I only had progression of liver mets after being off treatment for a month recovering from an op. Keep going! There's also been a case where a woman was given a full liver transplant in Leeds to cure her bowell cancer. There were news reports on it so worth a Google. There are things they can do!

They originally told us that liver transplant would be to consider as possible ‘cure’ but because his first chemo didn’t work they came back on it. We have been having private consultation as like you said your nhs oncologist is not always fully dedicated. We almost feel like they think that my partner is going to die anyway so it’s not worth trying.
The SIRT procedure would be in private sector and we hope it will work to the point that ablation can be done, probably still in private clinic as nhs wouldn’t do it with more than 5 tumours.
Hi, This is a long shot, but I recently went through a Radiotherapy treatment called MR guided MRIdian radiotherapy at the Cromwell in London. Its provided by GenesisCare.
I was also treated for liver lesion in Segment 7 appox 11mm that was not possible or easy to operate or Ablation the surgeon said.
I had 3 sessions on the MRIdian system that basically sends rays of guided radiation to the cancer whilst you hold your breath in.
I'm now praying it has destroyed the cancer. Results are in approx 10 weeks when they re scan fue to the scarring that occurs. I've also started on Filfori chemotherapy again as have spread in other areas too.
I wish you the very best and no harm in getting a second opinion privately. From what I was told there are sadly only 3 machines in the country, 1 in London, 1 in Oxford and Royal Marsden.

It sounds like similar treatment which my partner is going to have - targeted radiotherapy. We are all praying that it will do something as he has 30 tiny liver mets.
