Colon cancer stage 4 - Liver metastasis discovered- need advice please

Hi all, sorry for the long message, finally decided to join the group after being  diagnosed in 2020 with Colon cancer and went through emergency operation to get Sigmoid removed, had a stoma and then revrsed.

Diagnosis:Treatment:

07.06.2020: Resected Adenocarcinoma Sigmoid Colon Perforated tumour

Over the years had Oxaliplatin initially after the operation and then was ok for 2 years, and then unfortunately more was discovered and went through cheamo again but this time on Filfori and cetuximab in 2023 and 2024, had 6 cycles both times to battle cancer in lympth nodes.

Also went through MRIdian radiotherapy and stereotactic for a nodule in the Hernia that I have had treated went well thankfully. 

In the latest PET scan I had in June 25, and to my huge shock they have discovered a Liver Lesion of 15mm in Segment 7 and a nodule in the Mesentery which has grown slightly. I am completely devastated with the news !

Been told options of Liver Abalition and further treatment on Cheamo, but I've asked my Oncologist to discuss my case if I can have resection to give me a better chance ?

Can someone please kindly offer me some much needed advice on where I go from here ? Resection,  Abalition or Cheamo ?

    It’s always such a blow to discover more treatment is needed . However , I can share my mums teams approach to her spread to the liver . 
    She had five liver mets initially . The largest being 5 cm . She had chemo first and had a great response . She originally had 4 wedge resections and bounced back however the spread was a bit more imbedded than originally thought . She had a quick recurrence. Back to more chemo but this time she was sent  to  centre of excellence for the liver . They are able to do some of the more extensive operations. She had 73% of her liver removed . She did not have a future recurrence in her liver. For seven years she was cancer free then a small lung met removed.  It was a two day hospital stay . She had 15 years in total . Cancer did pop up again but it was age that prohibited more aggressive treatment . The first few years were the most intense in getting the disease under control but after that it was more like a chronic condition that required a bit of quick intervention now and again .

    The surgeons for the liver were highly skilled and  she felt very safe in their care .

    It’s great you have other options like ablation too . That’s wonderful and sometimes it’s good to go in person and meet each specialist. There has been a lot of success with ablation too . Less invasive but you have the options to investigate both .

    Sometimes getting a second opinion is really helpful . It gives space to think through all aspects . But what is very clear is you have good pathways . The oncologist are quite skilled at guiding . If there is more than one site they will consider what is the best approach in terms of intervention and speed of recovery if chemo is to be considered . Surgery can sometimes delay chemo so they weigh that up too . 

    Obviously only got my mums experience to compare but she only had three segments that were clear of cancer so you are definitely ahead of her only having one segment involved .

    Hope that encourages you . Totally understand getting weary with this but hopefully it will settle again for you .

  • I had two liver mets, and following chemo which successfully shrunk them, I had resection by keyhole surgery which meant a much quicker recovery. I wasn't offered ablation but that also sounds like a great option if surgery is deemed difficult.  A lot depends with the liver on where they are and how easy to get at. All the best for whatever you choose.

  • Hi all.. just a quick update from me to say that the Oncologist team has decided I should go with the MRI guided radiotherapy also known as MRIDIAN to treat the Liver metastasis and one in the masentry. 

    The surgeon said the liver met was deep between segment 7 and 8 and also said Ablation would also be very challenging procedure.

    Treatment will start in July so praying that this treatment works for me in the Liver and the noudle in the masentry. They said 5 sessions on each site on a daily basis and then rescan in 2.5 months time.
