Is stage 4 the end? What do we do now?

hi guys,

wishing everyone’s health and happiness. I wanted to give a bit of background so maybe I can get some help. My dad had some pain in his side last April. He’s a very healthy 71 year old - active and fit and never even been sick before. Perhaps the odd cold. Then he was diagnosed with stage 4 colon cancer in May. A shock. We were put onto a chemo path of - fluorouracil, folinic acid

On 13 November we had CT results back which showed that the cancer had spread. It had gone to the liver and to the lung. He had metals of 2cm.

We moved onto a new chemo - cetuximab and encorafinib started on 27 November 2024. This didn’t work either as the mets grew to 4cm. The mets in his liver are the biggest worry as they’re caused him significant pain and no quality of life.

Has anyone else had stage 4 colon cancer spread to the liver? Why isn’t it working? Has chemo worked for everyone else? Is liver ablation an option? I will try everything and anything honestly!! I really will. I cannot lose my dad so soon.

The team are looking to change to a third line of chemo called Oxaliplatin, fluorouracil, and folinic acid. Before this I just wanted to post on here and see if anything else might work for us before we dive into a new chemo. Thanks for reading. Please help if you can xx

  • Hi, I have stage 4, diagnosed aged 37, now almost 39. It's not the end, but sounds like your oncologist is trying to find a mix of chemotherapy that will get systemic control over the spread before any surgery. This is so you don't go through surgery and recovery just to find out it's come back the next month. It's all very tricky at stage 4. With me, they didn't want me on chemo while recovering from bowell surgery, because the chemo meds can stop wounds healing. And in the time I was recovering from surgery, it was spreading somewhere else. I've had liver surgery, only after targeted radiotherapy to shrink the liver mets. So radiotherapy might be worth asking about. Hopefully we can live a long time on stage 4. It's not easy and you can always ask for a second opinion and do some research about what other hospitals or doctors specialise in your specific case and ask for their opinion. 

    Everyone is different and immunotherapy may work for your Dad, it's down to certain genetic markers

    Some other threads have mentioned liquid biopsies for testing the tumour to get the most effect treatment. Don’t know if it’s private or effective. 
    Just a thought of you feel the current chemos are not achieving the desired results .
