Hi I had abo hysterectomy back on 10 Dec - all seems to have gone to plan and gynse consult said during telecon all cancer removed.
I am due a face to face check up in about 6 weeks.
However my discharge letter shows Substantial LVSIs. These were not mentioned during telecon and having never heard of them I googled. Google worried me so I phoned clinic and nurse explained that MDT had agreed no follow up treatment needed.
I am still concerned LVSIs are found in blood vessels and can lead to metastasis and will discuss at my face to face.
Meanwhile I wondered if anyone else on here had also been diagnosed with substantial LVSIs and if they had had any further treatment. Thank you.
Hi Judi
I hope that you are recovering well from your surgery.
Do you know what your stage and grade were post op? This influences further treatment.
I also had substantial LVSI. I was stage 1b (so contained) and had carcinoma sarcoma.
My follow up treatment was 4 cycles of chemotherapy and then 25 sessions of external beam radiotherapy.
Because of the LVSI I would be asking why further treatment has not been recommended? There may be a good reason for this however it would be worth asking for further info.
My chemo started 6 weeks after surgery and then the radiotherapy followed that. If further treatment is needed then it normally starts fairly soon so it is something I think needs clarifying.

Hi Judi
I had my hysterectomy in February 2024. I also had LVSI noted on my histology but it didn’t say substantial though, just >5 foci. I was stage 1a grade 1. The consultant offered me three options which was no further treatment, brachytherapy or external radiotherapy. I was given a couple of weeks to decide and in the end I went for the brachytherapy which the consultant agreed with. I had four sessions. Since then I have had regular follow up CT scans and all has been clear. My hospital offered scans and in person consultations whether I chose to have the brachytherapy or not which was very reassuring.
Originally my local hospital where I had my operation told me that I didn’t need further treatment before the results from the MDT came in, but then I was contacted by the specialist cancer centre where I was told LVSI had been identified and I transferred over to them for treatment etc.
Hope this helps and do ask what follow up monitoring is being offered.
I agree with Waidh about raising it sooner. If they do decide to offer adjuvant treatment, the NICE standard is to start within 12 weeks post op and it can take time to arrange.
Thank you - could I ask what NICE is please? Sorry if I sound stupid that I not know.
Hi Judi, it’s ok, many people don’t know. NICE stands for National Institute for Care and Excellence. They provide science based guidelines for the NHS.
Thank you - always good to have something to quote and know what I talking about. X
