Testicle Pain - Orchiectomy

Hi I am new here and was hoping to find some help and support regarding lost of a testicle. 

I was diagnosed with chronic testicle pain 14 years ago and have undergone numerous tests, scans, and surgery, but nothing has helped. 

My quality of life is extremely poor as I cant do the simplest of tasks because of the pain, the pain is in my right testicle and goes down my right leg. I cant stand or walk for long, I cant even sit down for too long as the pain gets so bad, I cant even wear jeans or trousers as the seam of the clothes triggers the pain as it rubs on the testicle.  

My urologist has now recommended Orchiectomy of the right testicle, and I was looking for some advice regarding peoples experiences with lost of one testicle. 

Have you guys had any long term pain from the surgery?

Have any of you guys gained weight, or lost muscle mass etc? 

Thank you for any help and advice you can offer me.

    Sounds very much like chronic prostatitis/epididymitis then mate. Have you tried flomax at all? I found it really helped but you do get some side effects. Sometimes taking flomax with antibiotics at the same time can give good results (coamoxiclav worked best for me).

    Look up prostatitis and flomax and also gluten. Going gluten free worked wonders for me The slightest intolerance can really inflame the prostate. If it turns out to be your prostate then the pain will be referred from that into the epididymis, unlikely to be the other way round. Physio can help aswell or milking the prostate. Have you tried any of that at all?

  • Hi mate, I've had my prostate checked a few times and all seems fine, also my PSA test came out low.

    My pain does feel like it's the epididymis that is hurting, seems to be the top and bottom of my testicle (head and tail) of the epididymis. 

    I don't seem to have any prostatitis symptoms, no penis pain, no pain when urinating or ejaculation, no back or anus pain, but I do wake up at night to urinate one to two times, but I've done that ever since i can remember. 

    If it's my epdidymis at fault ,then removing the testicle should work, if it's my prostate at fault what would removing my testicle do, would it still work to stop my pain?

    Not too sure mate. If you have prostatitis then removing the testicle might not cure the pain but your urologist sounds pretty confident, maybe he’s had success before? Does your testicle swell when you have pain and ride up higher or does it hang loose? I find mine only gives me pain when it contracts or rides up which is why I’m having my cremaster cut so it wont be able to pull up. It’s so uncomfortable having a tight scrotum 24/7. The only relief is having a hot bath or shower. Bowel movements or lifting heavy weights seems to cause me pain as well.

  • Hi bud, My testicle hurts at any position up or down, but I do get pain relief when I lay down though, the pain goes when I lay down. However! If I were to wear jeans or trousers 24/7, I would be in pain 24/7, certain pairs of jeans I have makes the pain worst.

    Do hot baths help? Another thing I’ve remembered is there’s a good forum post about this on patient.info. Search for “Has anyone had Microsurgical Denervation?”

    You’ll see a few posts from me on there but also many other people in a similar situation. Let me know what you think.

  • Hey mate, Where was the incision site done when you had the right testicle removed, was it lower down in the scrotum or higher up in the groin?

    Hi mate, it was through my groin because they thought I had cancer and wanted to get the entire spermatic cord which is the standard for cancer cases. By the way I’ve just seen my emails and another guy caller oscar18 just made a post about pain in his testicle on this site. On that patient.info site my name is Paul27735890, is yours “we5t”?

  • Hi mate, Yes my name is We5t over there, nice to meet you Paul. I will have to go and take a look at oscar18 post.

    I've been thinking about what my surgeon said last week, he reckons my best option is to have a Orchiectomy as it cuts everything, I guess its the subinguinal orchiectomy. He said he would most probably go in through the scrotum and cut high up as it has better results for scrotal pain, but my worry is... I don't think they cut the ilioinguinal nerve during an orchiectomy, what if my pain problem is the ilioinguinal nerve that is carrying my pain, if they don't cut this nerve I could still be in pain???

    This is the confusion bit, or unless I failed to understand what my surgeon said, he said that Microsurgical Cord Denervation doesn't always work, they cant make sure they cut all the nerves as some they cant even see which gets left behind. BUT! I've read that during the Microsurgical Cord Denervation procedure the ilioinguinal nerve gets cut and tucked away safely.

    So it seems the MSCD procedure cuts more nerves than Orchiectomy, as MSCD also cuts the ilioinguinal nerve, if I have my information correct. The only down side from MSCD is if they cut the vas, as this is basically like having a vasectomy, it can cause back pressure and make the epi congested. I've read they can strip back the outer layers of the vas to cut the nerves instead of cutting it totally, this then preserves the vas to avoid the epi getting congested.

    I would have thought my surgeon would rather try the MSCD procedure first before doing a Orchiectomy, I understand the MSCD procedure was invented to help preserve the testicle and to avoid going straight for Orchiectomy. I get ache and pain in both my testicles which also radiates down both legs, so I may end up needing both testicles removed, you would think they would try the MSCD procedure first???

    My surgeon seems such a lovely guy, he seems very caring and confident, I have no cause for concern really; but I'm so confused.
