I was on ADT for a year. PSA was in control for around 8 months and then started increasing. My doc ordered PSMA PET scan and it revealed 2 bone mets. Took SBRT targeted radiation. Then my doc prescribed chemo.
So I took 6 cycles of docetaxel. My PSA is ok as of now, 0.01.
The biggest side effect is hot sweats. What i now feel is much worse and severe than the hot flashes during ADT. Now throughout thr day there are multiple episodes of excessive sweating,. If tbere is someone out there who hasbtaken docetaxel chemo and has managed the excessive sweating with any medicinez please share.
But in my case now the prognosis is not good. Courtsey, the intraductal carcinoma detected in my ppst ops biopsy. They say if IDC is detected then the biology of the cancer is aggressive. There are already multiple clones of damaged genes, which will keep on mutating and rssisting treatments.
In my case, no treatment bas given me a long remission..radiation to prpstate bed dint work, ADT dint even work for 1 year. My doc says that though i have got a deep response to PSA suppression, the gain wont be long and my chemo may remian effecfive for about 6 to 8 months.
There is another anxiety in my case. My adenocarcinoma may be transitioningbto treatment induced nueroendocrine tumour. If that happens then i lose psma avidity and my future treatment options get limited.
Lu 177 gets ruled out. I am not MSI H so immunotherapy is also ruled out. As of now the only treatment left for me is cabazitaxel and lu 177 if i remain psma avid, and if my cancer morphs into t NEPC, then its platinum based chemo like carboplatin, which is basically a treatment for non small cell carcinoma.
Anyways, the best part is i am no more depressed and take each day as it comes and as a bonus. Worrying about the bleak future will spoil the present.
There is another decision i have taken, once all options get exhausted. Palliative treatment till its affordable and feasible, after that ..................
