Concerned now it's spread to lymph nodes in prostate anyone else in the same boat?

Hello

I was diagnosed with advanced prostate cancer stage three in 2018 was put on a clinical trial and had radiotherapy on the prostate and also on the pelvic bone. My PSA has stayed at a steady level until now.

It has reoccurred in my prostate the cancer, but is also gone to my lymph nodes in my prostate, and the clinical trial team have told me that the only treatment I can have now is hormone injections, and no more other treatment.

I just like to ask if anybody else has had the same symptoms and should I take a second opinion and what do you think the outcome might be as I’m very concerned.

    It's a difficult one when in the nodes.

    I always thought that if in a couple of nearby nodes then still potentially curable but distant nodes probably not.

    If in 5 or 6 nearby nodes then I have read negative posts.

    So possibly worth finding out exactly how many nodes and if Def nearby.

    If the case then yes second opinion privately can Def be worth doing

  • Hi  , welcome to our forum.  It might be useful if you could provide us with a little more history. PSA, staging and scan results etc.  None of us are medically trained so can only go on our own experiences and what we have picked up along the way, but the group on here are very good and have a massive wealth of knowledge.  I am confused that you have been told only one treatment available.  Did that refer to one treatment now or forever?  Most of us (long term’ers) on this journey have been through a variety of treatments and we are constantly being told there are more options in the toolbox, so I was surprised to read your comment.  If you want to see anyone’s journey, lots of us keep a diary on our profile, just click on their name or avatar next to the name.  Feel free to ask any questions.  David

  • Hello  and welcome. I am also surprised that you have been told that your only option is injected hormone therapy as my husband has had a range of treatments for advanced prostate cancer with several progressions along the way.

    What is available will be determined by the treatment you have already had and where. To help us help you can you answer the following questions.

    What hormone therapy have you had already and for how long.

    You have had radiotherapy. Do you know what dose you had and did it include the lymph nodes in the pelvic area.

    How quickly is your PSA rising and what level did it get down to after your initial treatment and what is it now.

    It may be that your experts are looking to see how the cancer reacts to the injections before they suggest a more systemic treatment like chemotherapy.

    Your health plus any co morbidities can also influence what treatment is recommended.

    Getting a second opinion from a centre of excellence could be a good idea.

  • Now I was diagnosed in 2018 with Advance Prostate cancer my PSA was 24.44 Prostate biopsy was Gleason 4+5 involving 6/6 cores in right lobe and 3/6 in left lobe with tumor of 14.1mm. 

    I was asked if I would like to go on a clinical trial which I said yes, this was inserting gold pellets around the tumor and having 6 weeks of radiotherapy and also radio on my pelvic bones to stop any trace of the cancer going there.

    My prostate can not be removed as the cancer has gone to the outer rim area. I also was on at the time and after Tamsulosin, Bicalutamide, Metocloramide, Prostap SR DCS 3.7mg injections.

    After all treatment my PSA levels from 2018 till 2022 was a great 0.07/0.12/0.28 until last year 2024 rise to 4.28 then last month (Feb 2025) I was sent for a PSAM

    Pet scan in Liverpool. Results show a slow growth back in the prostate but 2 very small tumors on the lymph nodes near the prostate, my PSA now is 2.98. I have now been told that any other treatment is not on the table the only treatment I may have now is hormone injections. I have a choice to make do I take the injections now and have all the side effects back form the hormone injections or wait and see if the tumor grow and then start on the hormone injections as it may be a couple of years before any growth or they may grow straight away? Well there you go has anyone had or are having the same dilemma to choice from and do I get a second opinion on can I have anymore treatment?

    I had prostate removed but found in lymph nodes after psma scan. Scan showed cancer in 2 nodes.

    Advised to have super extended pelvic lymph node dissection, ie All pelvic nodes removed. Cancer was found in 3 nodes in total out of I think 38.

    Do not notice any difference with them removed. If considering removal I would go for it.
