Lymph nodes removal

People can someone tell why aren't the positive lymph nodes removed ,they remove lots of them in surgery, why not the positive ones, just questioning.

    Generally if a T3 surgery is not normally done and probably especially if in some nodes, I believe you have about 5 nodes in question.

    So HT and RT best approach, still potentially curable but it is more tricky at this stage

    Taking out lymph nodes is normally connected with prostatectomy. While you’re doing major surgery anyway it’s easier to remove Lymph nodes as well. What you see on the PSMA Pet Scan is probably not the whole picture because of the possibility of Micro mets not showing up and if you do surgery then you still need to do whole pelvic radiation therapy after you’ve healed enough.

    I’ve heard of something they do in the Netherlands called ’ frozen section LND’ together or after prostatectomy.

    Then of course there have been trials doing prostatectomy with planned radiation afterwards and together with hormone therapy. But only trials / studies

    I really think you should try to focus on the possibility of getting the WPRT because most other things you’re looking into are more trials and / or studies with less evidence of success behind it.

    I think after careful consideration I am in agreement with  here.

    Invasive surgery can't be 100% sure that the entire cancer has been removed, and then you would still be heading down the HT/RT path.

    I really think you should try to focus on the possibility of getting the WPRT

    Stick with what has been proven to work and is tried and tested.
