Active Surveillance

Hi everyone, if you read my bio I had an MRI that showed a 14mm peripheral zone lesion. This led to a 15 core biopsy . Only one core found very low volume Gleason 3+3=6 in a different part of the prostate.

So I’ve been put on A/S quarterly psa. Checks and a MRI in 9 months. My consultant has set my psa at 6.5 for any recall which I feel ( not sure why) is quite low. previous psa were 3.7 then repeat 5.04. 

I just don’t know how to feel about it, sometimes I’m happy and relieved other times I question the biopsy considering it was a pirads 4 lesion. They’ve told me the biopsy targeted the abnormality and nothing was found there, maybe I just have a trust issue. Has anyone else had similar and how they felt about it??

    The only thing I would be asking is how near to gland edge is 14mm tumour although they must realise it's ok , still worth checking.

    I had a 13. mm tumour which was near the gland edge so started treating but then positioning may  have been different.

    Just a quick one, you have had a biopsy, we have your PSA and Gleason Score - do you also have the TNM staging?

    i am aware that your feelings about being on AS are "all over the place" and i assume this is due to the PTSD. I am also aware that you may be thinking that some action will be needed to deal with the cancer in the future. I hope you don't mind but may I make 2 suggestions:-

    * You return to the Oncology team (speak to your Macmillan Nurse or CNS - Cancer Nurse Specialist) and tell them of your PTSD and you have anxiety about the rising PSA.

    * Give our support line a call on 0808 808 00 00 (8am to 8pm 7 days a week) let them know of your history of PTSD and have a chat with them.

    Same happened to me. An MRI showed a PIRAD-4 lesion and subsequent biopsy showed 1 core with Gleason 6 in a different area of the prostate 

    I've been on active surveillance for nearly 8 years.

    However, a PIRAD score just indicates the probability of there being a clinically significant disease.
