Excision margin- margin not reached meaning?

On the pathology report it says

excision margin: margin not reached 


 Confused what it’s referring as to as normally it should say positive or negative margin? 

  • Hi Daisy03 welcome to the forum. Normally there is a margin  that they like to  have when excising any lump etc and it looks like they didn't get that margin thta they were looking for or as they would have wanted. In the case of a Lumpectomy this would usually mean more surgery to get the margins they need. 

    Hope that makes sense?

  • Thank you for your reply. I’m really confused as on the post op appointment the surgeon said he removed all the cancer. 
    he didn’t say another surgery will be required

  • I had lumpectomy in July which removed the tumour successfully, however when the look at the tumour more closely the look at the margin of tissue around it. I didn't have a clear margin which meant having a local excision to take another small amount but still no clear margin. I have a choice of further local excision or mastectomy. It's important to remember that the margins have pre cancerous cells but obviously they can be concerning xxxx

  • Im sorry you have to go through that multiple times. My mum did have mastectomy. Im just confused as no one mentioned this until I read the report. And it doesn’t say clear margin/ negative margin - like it would normally say. Just says not reached which is confusing. It says nearest margin is  4.5 mm from inferior margin.L underneath it too

  • 4.5mm is much more than the required 2mm margin so I it sounds like it's a good margin which the cancer did not reach. X

  • I would read it as, "the caner did not reach the margin," which would align with what your surgeon has said, but that is a guess. Best to contact the surgeon's secretary and request clarification.

    Still unsure but finally spoke

    to the nurse and said she thinks it’s all clear. Will hopefully ask oncologist when we see him 

  • Hi hope you are feeling better after speaking to your nurse, I had two lumpectomys on right side,thankfully got the margin second time, just finished my radiotherapy and hopefully all good, I hope everything goes well for you, I will keep my fingers and toes crossed for you
