Help - What to do Surgery or Wait and See!????

4 weeks ago, after positive FIT test I had Coloscopy when 15mm tumour was found and removed, biopsy showed cancer in tumour. CT Scans all clear, it hasn't spread and no lymph nodes involved. . So Im T1,NO,MO but I don't yet know what grade the cancer is.   They got the tumour out in a few pieces, he said at the time he thought he'd got it all out but cant be sure so I would need another colonoscopy in 3 months time (that was before biopsy showed it was cancer)

Im meeting with Colorectal surgeon on Monday to discuss diagnosis and treatment.

Ive been reading a lot about mid transverse colon cancer, and its got me very worried:-

  • Surgical options
    Surgical options include extended right or left hemicolectomy, or transverse colectomy. Transverse colectomy is considered a challenging procedure due to the need to mobilize both flexures and dissect around the middle colic root. 
  • Prognosis
    Mid-transverse colon cancer is associated with a poor prognosis. For example, patients with stage I to III mid-transverse colon cancer have significantly worse 5-year overall and disease-free survival rates than patients with right or left colon cancers. 

So for Monday I understand they are going to advise on 2 options 

1) I can have part of my bowel removed not sure how large a section  as above. My tumour was in mid transverse section and right in the middle of top horizontal stretch, which I understand is not common and can be difficult to remove.  Added to this I have no gall bladder and already suffer from digestive problems Bile acid malabsorption (BAM)  Frequent, urgent, or unpredictable bowel movements 

Or  2) continue with monitoring regular/frequent colonoscopies  3 , 6, 12  months and CT Scans.

It seems to me that everyone goes for the surgery option - I can understand why if this ensures all cancer is removed but it seems so drastic for someone who is at stage 1 without any signs of cancer spreading.  5 years ago I was diagnosed with Bladder Cancer was Stage 1 non invasive but Grade 3 fast spreading. I had tumour removed and then 5 years of BCG instillation treatments which were successful and cancer did not return.   I know that bowel cancer is different but it would appear that it has been caught very early again so is radical surgery necessary. 

Im 72 years of age overweight and not that fit, worried about surgery complications, recovery.  I will of course discuss all this with the surgeon on Monday but in the meantime would really like to hear from anyone - is there anyone?  who was in same situation and diagnosis as me and took the No Surgery - Wait and See route.

  • Hi Wendyseahorse welcome to the forum. I can understand why you have a dilemma, but the eventual choice has to be yours, based on the best available evidence. I can't answer your query fully as my Cancer was a different type but I feel sure some of the lovely folks on here will be along soon to offer some thoughts. I think it is sensible to have that conversation with the Consultant and you sound better informed to make whatever choice is right for you..x

  • As granny59 says, the final decision is yours. However, I am curious why they aren't offering chemo to mop up any remaining cancer. I'm no doctor so there may be a good reason it isn't offered. However, if they haven't explained that to you, then you may wish to ask. 

  • Thank you for replying Susan....they haven't mentioned chemo mop up but Ive only spoken to nurse who gave me the feedback from the MDT meeting and then made an appt for Monday.  It may well be an option.  I really wanted  to hear from anyone who had tumour removed and didnt go for surgery, but Ive had no responses yet... also asked this on Bowel Cancer UK forum and FB...... Perhaps these people do not use support forums?
