Hi, my cousion brother suferring from rectum cancer. Initially chemothropay is done. but it was not srinked the tumor. Later radiation along with chemothropay is done. Than also there is no improvement. Later now, doctor tried to operat through APR. But it was informed that unable to revome or operate. But they have done laprospy with colostomy diversion. But tumor is not reomved. What will be the next step? His age is 27 years old and he is unmarried.

That is so terribly sad . I am so sorry for him . Do you know his cell type? . They may try adding in other agents depending on his cell type . Then try second line chemo . Some people who have not responded in the way they would have liked can sometimes try second line chemo and people have come through here have reported some better responses. They will talk him through all his options . Bowel cancer has a few different chemo combinations .
It is a lot for him and the family to endure though .

They have informed that they will start 2nd type of chemo. Could you tell me posibility/probability of shrinking the tumor.
They have informed that they will try 2nd and 3rd type chemo. If tumor shrinks, then they will go for surgery for revomal of tumor from rectum.
Kindly advise and through light on next treatment

I am so sorry I did not get back to you . I have to say I don’t know if it’s stress from all the CV news but my memory has been awful . I hope you can accept my apology.
If he has had a biopsy of the rectal tumour they may be able to tell you if he is MSI or MSS .If he is MSI then immunotherapy treatment is sometimes available on clinical trials . You might like to read up on that for future consideration. It’s hard to get a clinical trial with it just now and Corona virus has interrupted everything but America has it as part of their first line treatment now . Maybe something for the future .
I honestly don’t know the percentages or how much shrinkage as it really is individual but I find Pubmed a good search engine for research .
However there have been people who have not had the desired outcome and achieved it with second line chemo . You also might like to look at a second opinion at some point for another surgeon . Some can do more extensive surgery than others .
I wish him all the very best and hope he gets the shrinkage he needs to get him to surgery .
