Query - Stage 3 Bowel Cancer and Rising CEA during Chemotherapy

My husband has just started his fourth cycle of CAPOX (oxaliplatin and capecitabine), having been diagnosed with Stage 3 bowel cancer in March.  He had very low tumor markers when he started the chemotherapy in early May -- i.e. CEA and CEA 19-9 that are checked.  After the first cycle / before the second cycle, his tumor markers increased slightly.  They have continued to rise slightly between second and third; and, now the third and fourth cycle.  His readings are now 3 for CEA and 15 for CA 9-9.  I'm concerned, and while we are due to speak with the oncologist in a couple of weeks when my husband completes cycle 4, I am wondering if anyone in the Macmillan Cancer Support network has any experience with this sort of thing and can give insight.  I will welcome anything, please. Thank you in advance!

  • Hi

CEA markers are more reliable off chemo than on chemo apparently. Chemo itself can cause it to rise . It’s still very low and I am sure your oncologist would intervene if they were concerned at all.

It is also normal for a bit of fluctuation to occur. Have a chat with his team and let them reassure you . It happened every time my mum was on chemo .

  • Thank you so much for your reply,  .  I really appreciate you for writing this response.  It calmed me.  We will find out whether there is implication or not over the coming several weeks, post my husband’s post cycle four chemo CT CAP and bloodwork.  I have been working the mindfulness to stay in the ‘here and now,’ and not ‘predict’ what will be!

Take care, too, Cour.  Regards.  Anna 2025.

  • Hi   Although the link below is quite old now and may be a little out of date in regards to follow ups and such like, it was written by a support nurse and covers rises and causes of CEA levels?

If you click on my name then my profile page will show that my CEA fluctuated between ‘less than 1’ and 1.8 even during my 5 years post treatment monitoring and my nurse said it can vary just like your blood pressure does.

Hope this helps and your hubby is tolerating the chemo ok

    Take care
